UnitedHealthcare medical policy (New Mexico only) defining medical necessity criteria for home sleep apnea testing (HSAT), attended full-channel polysomnography (PSG), daytime sleep studies (MSLT, MWT), PAP titration, repeat testing, and exclusions such as actigraphy and certain PSG indications.
Added notation that performing a repeat HSAT is not recommended when an initial HSAT is negative, inconclusive, or technically inadequate; recommend attended PSG after a single non-diagnostic HSAT.
Revised coverage criteria for attended full-channel polysomnography for suspected OSA to require prior HSAT results be within the past 12 months when negative, indeterminate, or technically inadequate.
Replaced MSLT language to state MSLT is medically necessary for evaluating suspected narcolepsy when other causes of excessive sleepiness have been excluded by appropriate clinical assessment.
Clarified repeat attended PSG and repeat PAP titration are medically necessary for individuals with persistent/new symptoms despite documented treatment and for those with clinically significant weight change (≥ 10%) or changes in cardiovascular disease since the last study.
