Home Sleep Apnea Testing (HSAT) - Medical Necessity

HSAT is medically necessary for evaluating adults with suspected OSA:

HSAT is medically necessary for evaluating adults with suspected Obstructive Sleep Apnea (OSA)

Adequate HSAT = minimum 4 hours and includes nasal pressure, chest and abdominal respiratory inductance plethysmography, and oximetry.

Attended Full-Channel Polysomnography (PSG) - Medical Necessity

Attended PSG is medically necessary for evaluating individuals with suspected OSA when ANY of the following are met:

Results of previous HSAT are negative, indeterminate, or technically inadequate to make a diagnosis of OSA

Individual is a child or adolescent (i.e., less than 18 years of age)

Known comorbid medical conditions that prohibit HSAT (ONE of)

Background & Evidence

Background: HSAT and PSG are distinct diagnostic approaches. HSAT refers to unattended portable monitoring that does not determine sleep stage and must include a minimum of 4 hours of adequate recording with sensors such as nasal pressure, chest and abdominal respiratory inductance plethysmography, and oximetry. PSG (polysomnography) is an attended, laboratory‑based study using multiple channels to record brain activity, eye movements, breathing, oxygenation and other physiological signals. Device classifications (AASM/Collop) group systems as Type 1 (full attended PSG ≥7 channels), Type 2 (full unattended PSG ≥7 channels), Type 3 (limited channel devices, ~4–7 channels) and Type 4 (1–2 channels, often oximetry). Rationale for home versus in‑lab testing includes similar diagnostic and outcome performance in selected, uncomplicated patients with high pretest probability of OSA, lower resource use for HSAT, but greater need for PSG when comorbid cardiorespiratory disease or other conditions exist or when HSAT is non‑diagnostic. Diagnostic thresholds follow AASM guidance: OSA is confirmed if obstructive events are > 15 events/hour without symptoms or > 5 events/hour with symptoms; severity is mild AHI/RDI ≥5 and <15, moderate ≥15 and ≤30, and severe >30. To rule out Central Sleep Apnea prior to implantation and/or calibration of an implantable hypoglossal nerve stimulator

Medical necessity criteria for daytime tests:

- An adult who is unable to stay awake, resulting in a safety issue

Attended PAP Titration - Medical Necessity

When attended PSG criteria are met, the following are medically necessary:

A split-night sleep study in a healthcare facility or laboratory for diagnosis and PAP titration

A full night study for PAP titration when a split-night study is inadequate or not feasible and the individual has a confirmed diagnosis of OSA

Attended Repeat Testing - Medical Necessity

Repeat attended PSG and repeat PAP titration are medically necessary when ALL of the following apply:

Individual has persistent or new symptoms despite documented appropriate current treatment or PAP therapy

Problems such as equipment failure, improper mask fit, pressure leaks, unsuccessful titration, inadequate pressure, and medical problems (e.g., nasal congestion) have been addressed and appropriately managed

Follow-up PSG/HSAT recommendations (AASM Caples et al., 2021)

Follow-up PSG or HSAT is recommended/used in the following clinical circumstances:

Reassessment of patients with recurrent or persistent symptoms despite good PAP adherence

Assess response to treatment with non-PAP interventions

Clinically significant weight gain or loss since diagnosis or initiation of treatment

Reassessment of sleep-related hypoxemia and/or sleep-related hypoventilation following initiation of treatment for OSA

Patients being treated for OSA who develop or have a change in cardiovascular disease

Patients with unexplained PAP device-generated data

AASM: MSLT/MWT indications and limitations

MSLT is indicated as part of evaluation to confirm narcolepsy

MSLT may be indicated to help differentiate idiopathic hypersomnia from narcolepsy

MSLT is not routinely indicated in initial evaluation/diagnosis of OSA or assessment after nasal CPAP

Use clinical judgment for SDB treatment prior to PSG-MSLT testing in pediatrics

For children and adolescents, clinicians should use clinical judgment to determine whether treatment for sleep-disordered breathing is indicated before performing PSG followed by MSLT testing. Consider treating identified SDB prior to proceeding with PSG-MSLT in pediatric patients.

HSAT negative/inconclusive result handling

If a single HSAT is negative, inconclusive, or technically inadequate and clinical suspicion for OSA remains, in-laboratory PSG is strongly recommended rather than repeating HSAT. Repeating HSAT instead of proceeding to PSG may risk diagnostic failure and could result in denial of the definitive diagnostic pathway.
