Coverage Criteria — When Imaging is Supported

General coverage criteria for advanced imaging

Covered when ALL of the following are met

Documentation and clinical evaluation: Recent detailed history and physical exam since onset or change in symptoms, and/or relevant labs and prior imaging are submitted to establish medical necessity.

Condition-specific sections may add requirements (e.g., spine/MSK x-rays).

Modality appropriateness: Selected imaging modality (ultrasound, CT, MRI, PET) is appropriate for the clinical question and less-invasive or lower-cost options have been considered when adequate to answer the question.

Appropriate use of contrast determined by indication.

Age routing: Individuals 18 years or younger follow Pediatric Imaging Guidelines when applicable; adults follow the General Imaging Guidelines.

Prerequisite studies: When indicated by the condition-specific guidance, a recent chest x-ray or other baseline study should be documented prior to advanced chest imaging.

Exceptions listed in condition-specific sections (e.g., supraclavicular lymphadenopathy, known bronchiectasis, suspected ILD).

Impact on management: Repeat or advanced imaging is only covered when results are expected to affect clinical management, demonstrate progression or recurrence, or meet listed condition-specific indications.

Avoid unnecessary duplication; prior imaging should be reviewed before ordering additional studies.

3D Rendering Coverage Criteria

3D rendering (CPT 76376/76377) may be considered when:

Clinical scenarios: Used for complex pre-operative planning or evaluation when conventional imaging is insufficient (examples include complex fractures/dislocations, spine/pelvic/acetabulum fractures, complex facial fractures, congenital skull abnormalities in infants, cerebral angiography and selected pelvis/abdomen indications).

Supervision requirement: Physician concurrent supervision and active participation in the 3D reconstruction process must be provided and documented.

Concurrent supervision defined by active design/monitoring of reconstruction (ACR recommendations).

Coding restrictions: Do not bill CPT 76376/76377 in conjunction with excluded modalities (e.g., CAD, MRA, CTA, nuclear SPECT, PET/PET‑CT, mammography, breast MRI/US, CT colonography, cardiac CT/MRI or coronary CTA).

Providers may be required to obtain prior authorization for these 3D codes even if the base modality is not subject to authorization.

Imaging Guidance Coverage Criteria

Imaging guidance codes are appropriate when ALL of the following are true:

Guidance purpose: Imaging is necessary to guide a needle, catheter, electrode, or device for percutaneous procedures (biopsy, drainage, ablation, localization).

Guidance codes include the imaging and radiologic supervision/interpretation for the procedure.

Appropriate code pairing: Do not routinely bill a separate diagnostic procedure code in conjunction with a guidance procedure; use the guidance CPT that represents the imaging portion of the procedure.

Examples and code table provided in guidance (77011, 77012, 77013, 77021, 77022, 76942, 75989).

Unit of service: Only one unit of a guidance code (e.g., 77012, 77021, 76942, 77013, 77022) should be reported per individual encounter/date of service.

Unit of service is the encounter, not number of lesions or localizations.

Unlisted Procedure Usage Criteria

Unlisted procedure codes (CPT 76497, 76498, 78999) may be used when:

No specific CPT exists: An appropriate anatomic site–specific CPT code or Category III code does not describe the performed diagnostic or interventional CT, MR, or nuclear medicine procedure.

Use unlisted CT/MR/nuclear codes when thinner cuts, different positioning, or non-standard acquisition is required beyond available codes.

Illustrative examples: Scenarios include navigation/planning for neurosurgery (Stealth/Brain Lab), custom joint arthroplasty planning, or navigational bronchoscopy planning requiring acquisition parameters not available in standard diagnostic codes.

When a Category III code exists it should be used instead of an unlisted code.

Limited CT (CPT 76380) Criteria

Limited or follow-up CT (CPT 76380) is appropriate when:

Use cases: Used for limited protocols such as limited sinus CT, limited/follow-up slices through a known pulmonary nodule, limited slices to assess a non-healing fracture, or other situations where full diagnostic CT work is not performed.

Do not use 76380 for treatment planning purposes.

Billing restrictions: Do not report CPT 76380 in conjunction with other diagnostic CT codes to cover 'extra slices' in an imaging protocol; AMA CPT defines no minimum/maximum number of sequences for CT codes.

Overuse or misuse (e.g., substituting 76380 when full diagnostic study was performed) may trigger coding review.

Modality-specific coverage guidance

Coverage and appropriateness guidance present in these preface sections:

Whole-body CT/MRI stance: Whole-body CT for asymptomatic screening is not indicated; WBMRI is generally not supported except for select cancer predisposition syndromes; WBMRI reporting should use CPT 76498.

Whole-body low-dose CT is supported for multiple myeloma staging per oncology guidance.

PET‑MRI is generally not supported but may be used in select circumstances when PET‑CT criteria are met, PET‑CT is unavailable, and the provider requests PET‑MRI; report PET‑MRI as CPT 78813 + 76498.

Quantitative MR analysis: Category III quantitative mpMRI procedures (0648T/0649T/0697T/0698T) and volumetric brain MRI codes (0865T/0866T) are considered investigational/experimental for routine clinical practice.

Prerequisite clinical evaluation

General prerequisites before advanced chest imaging

Pertinent clinical evaluation required: A pertinent clinical evaluation since onset or change in symptoms (detailed history and physical exam) is required prior to considering advanced imaging.

Other elements may include basic labs and prior imaging; a recent chest x‑ray over-read is often expected.

Components of evaluation: Evaluation should include a detailed history and physical exam, appropriate laboratory studies, and basic imaging such as plain radiography or ultrasound; a prior meaningful technological contact may suffice for established individuals.

Condition-specific sections may require additional information.

Chest x-ray guidance and exceptions

Chest x-ray role and exceptions

Role of chest x-ray: Chest x-ray is the usual initial imaging to identify disease and direct advanced imaging for pneumothorax, pneumomediastinum, fractures, chest wall mass, infections and malignancy.

Chest x-ray often expected before CT in many condition-specific pathways.

Exceptions to preliminary x-ray: Preliminary chest x-ray is not required for supraclavicular lymphadenopathy, known bronchiectasis, suspected interstitial lung disease, positive PPD/tuberculosis concerns, or suspected pulmonary AVM.

Condition-specific guidance defines exceptions.

CT and navigational bronchoscopy criteria

CT and navigational bronchoscopy imaging guidance

CT indications and options: CT Chest with contrast (71260) is appropriate to evaluate intrathoracic abnormalities detected on prior imaging; CT without contrast (71250) is acceptable when contrast contraindicated or for nodule follow-up; HRCT should be coded using appropriate 71250–71270 codes.

Low-dose CT (71271) used for lung cancer screening when indicated.

Navigational bronchoscopy CT requirements: A thin-cut, properly formatted CT chest with optimized reconstruction parameters is required for navigational bronchoscopy; a prior CT may not be usable if not formatted correctly, and CPT 76497 or 71250 are referenced depending on timing.CT within 6 weeks vs >=6 weeks

HRCT coding note: High-resolution CT should be reported with appropriate CPT (71250–71270); the additional high-resolution slices are not separately billable.

Modality indications

Modality-specific indications

CTA Chest (71275) can be considered for suspected pulmonary embolism and thoracic aortic disease and for pre-procedural vascular planning.

MRI Chest is infrequently indicated and may be used to clarify equivocal findings, to evaluate mediastinal or chest wall lesions, or when CT contrast is contraindicated.

Nuclear pulmonary imaging: Pulmonary perfusion and ventilation-perfusion studies (78580, 78582) and quantitative perfusion/ventilation studies (78597, 78598) are used for PE evaluation and pre-operative assessment when indicated.

Supraclavicular lymphadenopathy — Initial imaging

Covered when ALL of the following are met

Supraclavicular initial imaging: Ultrasound (CPT 76536) is the initial study for palpable or suspected supraclavicular lymphadenopathy; ultrasound-guided core needle biopsy (CPT 76942) may be performed when indicated; CT Neck with contrast (70491) or CT Chest with contrast (71260) is indicated if ultrasound is indeterminate.

Document prior ultrasound and findings.

Axillary lymphadenopathy — Localized

Covered when ALL of the following are met

Axillary localized initial evaluation: Axillary ultrasound (CPT 76882) is the initial evaluation for any axillary mass or enlarged node; consider a 3–4 week observation for benign clinical presentation (12+ weeks observation for ipsilateral COVID-19 vaccine–related adenopathy); perform ultrasound-guided core needle biopsy or excisional biopsy if nodes persist or malignancy is suspected.3-4 weeks or >=12 weeks for vaccine-related

No advanced imaging indicated if biopsy is benign.

Axillary lymphadenopathy — Generalized

Covered when ALL of the following are met

Axillary generalized evaluation: Axillary ultrasound (CPT 76882) for initial evaluation of generalized axillary lymphadenopathy; proceed to ultrasound-directed core needle biopsy or surgical excision if persistent or if malignancy is suspected; perform diagnostic work-up (including serology) for systemic causes.

Advanced imaging generally not indicated unless pathology or clinical course warrants.

Mediastinal lymphadenopathy — Imaging and follow-up

Covered when ALL of the following are met

Indication for CT chest: CT Chest with contrast (71260) is indicated for mediastinal abnormalities detected on chest x‑ray or other non‑dedicated imaging or to clarify mediastinal findings on non‑contrast CT.

Document lymph node short-axis measurements.

Follow-up for nodes >=15 mm: Follow-up CT Chest (71260) at 3–6 months is recommended for mediastinal lymph nodes with short‑axis ≥15 mm when there are no other thoracic abnormalities; stability or decreased size after follow-up obviates further advanced imaging.>= 15 mm

If persistent/increasing size or suspicion for malignancy, consider biopsy or PET/CT.

Further evaluation triggers: Persistent or increasing lymphadenopathy on follow-up CT, suspected malignancy, or unexplained disease should prompt lymph node biopsy or PET/CT as appropriate.

Refer to oncology guidelines for lymphoma workup.

Cough — Imaging pathway

Covered when ALL of the following are met

Initial evaluation should include a recent chest x‑ray obtained after the current episode of cough started or changed; discontinue medications known to cause cough (e.g., ACE inhibitors).

CT chest for non‑smoker chronic cough: For non‑smokers, after a 3‑week sequence including antihistamine/decongestant or intranasal glucocorticoid, spirometry/PFTs, empiric corticosteroid trial and/or leukotriene antagonist, and GERD treatment, CT Chest (71260 or 71250) may be considered if chest x‑ray is normal and symptoms persist.3-week trial

Smoker or changed cough: Current or former smokers with new cough >2 weeks or changed chronic cough may proceed to CT chest earlier if chest x‑ray is abnormal or non‑diagnostic.>2 weeks

Refer to hemoptysis section when applicable.

Non-Cardiac Chest Pain imaging criteria

Covered when ALL of the following are met

Initial evaluation for non‑cardiac chest pain: Initial evaluation should include a chest x‑ray prior to advanced imaging.

If x‑ray abnormal, proceed to CT chest with contrast (71260) or CTA (71275) as appropriate.

Substernal chest pain after normal x‑ray: If x‑ray is normal and pain is substernal, perform cardiac evaluation OR GI testing/treatment OR PFTs prior to advanced chest imaging; CT Chest with contrast (71260) may be appropriate if non‑imaging evaluations are inconclusive and symptoms persist.

Non‑substernal chest pain after normal x‑ray: If x‑ray is normal and pain is non‑substernal, CT chest with or without contrast (71260 or 71250) and/or bone scan is appropriate for suspected malignancy, infection/inflammation, or prior chest intervention.

MRI chest (71552) may be appropriate for necrotizing fasciitis or surgical planning.

Dyspnea/Shortness of Breath imaging criteria

Covered when ALL of the following are met

Dyspnea initial imaging: Initial evaluation should include a recent chest x‑ray prior to advanced imaging.

If x‑ray is abnormal proceed to CT chest without contrast (71250).

Dyspnea when x‑ray indeterminate: If chest x‑ray is indeterminate and ECG/echocardiogram (or stress testing) plus pulse oximetry and PFTs are indeterminate, CT chest without contrast (71250) including HRCT or CT chest with contrast (71260) is appropriate.

If PE suspected follow PE guidance.

Costochondritis / Musculoskeletal chest wall imaging criteria

Covered when ALL of the following are met

Costochondritis imaging: Costochondritis or other musculoskeletal chest wall syndrome does not require advanced imaging (CT or MRI) unless other guideline criteria are met.

Diagnosis is largely clinical; imaging considered if dyspnea, trauma, or other concerning features present.

Pre-Operative / Pre-Interventional Assessment

Covered when ANY of the following are met

Pre‑interventional imaging options: Split function (quantitative perfusion) studies (78597/78598) or SPECT/CT (78830), OR CT Chest (71250, 71260, 71270) for pre‑interventional assessment prior to planned segmental/lobar resection or endobronchial valve placement.

Selection based on procedure type and clinical need.

Post Endobronchial Valve (EBV) Placement - Suspected Complication

Covered when ALL of the following are met

Post‑EBV initial evaluation: Initial evaluation for suspected post‑EBV complication should include a recent chest x‑ray.

Post‑EBV CT modality: CT Chest without contrast (71250) OR CT Chest with contrast (71260) is appropriate for acute loss of benefit, increased dyspnea, sudden chest pain, increased cough, suspected valve malposition/migration, or to evaluate target lobe volume reduction.

CT sometimes performed routinely at 6–8 weeks at some centers.

Hemoptysis

Covered when ALL of the following are met

Initial hemoptysis after chest x‑ray: Following a chest x‑ray obtained after hemoptysis started or worsened, CT Chest with contrast (71260) or CTA Chest (71275) is indicated.

CT without contrast (71250) only for severe contrast contraindication.

Recurrent hemoptysis: For recurrent hemoptysis after medical therapy or embolization, CTA Chest (71275) is indicated.

Bronchiectasis

Covered when ANY of the following are met

HRCT indications: High‑resolution CT Chest without contrast (71250) to confirm suspected bronchiectasis after initial x‑ray; for known bronchiectasis with worsening symptoms or worsening PFTs; or for hemoptysis with known/suspected bronchiectasis.

CT with contrast may be used for suspected acute infection complications or PE considerations.

Adult Cystic Fibrosis

Covered when ANY of the following are met

CF CT indications: CT Chest without contrast (71250) or with contrast (71260) for suspected/initial diagnosis of cystic fibrosis; routine surveillance biennially; persistent symptoms with reduced lung function despite therapy; exacerbations when chest x‑ray indeterminate; hemoptysis; suspected fungal pneumonia; and pre/post‑lung transplant evaluation.

Routine surveillance typically every 2 years using ALARA radiation doses.

Cystic Fibrosis — CT indications

CT Chest (71250 or 71260) is indicated for adult cystic fibrosis when any of the following apply:

CF CT indications list: Suspected/initial diagnosis; biennial surveillance; persistent respiratory symptoms with reduced lung function despite therapy; exacerbations when chest x‑ray indeterminate; hemoptysis; suspected fungal pneumonia; pre/post‑lung transplant evaluation.

Follow ALARA dosing; routine short‑term CT follow‑up during exacerbations not recommended due to radiation risk.

Bronchitis — imaging

Chest x‑ray and CT use for acute bronchitis:

Bronchitis imaging pathway: Chest x‑ray is indicated as the initial imaging; advanced imaging is not needed for acute bronchitis unless directed by condition‑specific guidance (e.g., to rule out pneumonia).

Obtain chest radiograph for abnormal vitals or lung exam.

Asbestos exposure — imaging

Asbestos exposure imaging rules:

Asbestos imaging: Chest x‑ray is used for radiographic screening; stable calcified pleural plaques on chest x‑ray do not require advanced imaging; CT Chest should not be used to screen at‑risk populations; HRCT (71250) is indicated for any change on chest x‑ray or progressive respiratory symptoms suggesting interstitial fibrosis.

COPD — imaging

COPD imaging criteria:

COPD CT indications: Chest x‑ray initially; CT Chest (71250 or 71260) can be performed for suspected/known emphysema when pre‑operative LVRS is requested, when diagnosis is unclear after PFTs and chest x‑ray and alternate diagnoses are suspected (e.g., bronchiectasis, sarcoidosis, pneumoconiosis, ILD), or for pre‑interventional lung procedure assessment (eg endobronchial valve) or transplant evaluation.

Select modality based on clinical question.

Interstitial Lung Disease / Diffuse Lung Disease – Indications

HRCT (CPT 71250) or CT chest (CPT 71260) is covered when ANY of the following indications are met:

ILD imaging indications: Interstitial changes on other imaging; pulmonary symptoms with abnormal PFTs and normal chest x‑ray but high clinical suspicion for ILD; initial imaging in connective tissue disease or significant exposures; detection of Progressive Pulmonary Fibrosis (new/worsening symptoms or PFT decline); and annual imaging in known pulmonary fibrosis if clinically indicated.FVC decline >=5% or DLCO decline >=10% within past year for PPF

HRCT preferred; include inspiratory/expiratory and positional views when appropriate.

EVALI

CT chest (with or without contrast) is covered when EVALI is suspected:

EVALI: CT Chest (71250 or 71260) is indicated when EVALI is suspected based on clinical presentation or when chest x‑ray is non‑diagnostic and further characterization is required.

Chest x‑ray may be used initially; CT used for further evaluation.

Pneumonia

Pneumonia imaging pathway:

Pneumonia initial and advanced imaging: Chest x‑ray should be performed initially for suspected pneumonia; CT Chest (71250 or 71260) is indicated if chest x‑ray shows complications (abscess, effusion, necrotizing pneumonia, pneumothorax), if associated with possible lung mass, for hypoxia/respiratory distress, abnormal vitals/exam, advanced age (>75) or significant comorbidities, or for immunocompromised patients with high suspicion or persistent/diffuse radiographic abnormalities.

If PE suspected follow PE guidance.

Pneumonia — CT Chest indications

CT Chest (71250 or 71260) is covered when ANY of the following are met after initial chest x‑ray:

Complication of pneumonia: CT Chest (71250 or 71260) when chest x‑ray demonstrates complication of pneumonia such as abscess, effusion, necrotizing pneumonia, or pneumothorax.

Possible associated mass: CT when a possible lung mass associated with the infiltrate is suspected.

Hypoxia/respiratory distress: CT for hypoxia and/or respiratory distress.

Negative/equivocal x‑ray with abnormal clinical features: CT after initial chest radiograph is negative/equivocal AND one of: abnormal vital signs (hypoxemia, pulse >100, RR >24, fever >100), abnormal exam (respiratory distress, dyspnea, abnormal lung auscultation), advanced age (>75) or significant comorbidities.

COVID-19 — CT Chest indications

CT Chest (71250 or 71260) is covered in symptomatic or post‑diagnosis scenarios when imaging will affect management:

High‑risk symptomatic COVID: Symptomatic COVID‑19 positive individuals with underlying comorbidities (including age >65 and listed comorbidities) where imaging will affect management.

Moderate to severe disease: Moderate to severe symptomatic individuals with significant pulmonary dysfunction (eg hypoxemia, moderate‑to‑severe dyspnea), regardless of test results or when testing unavailable.

Worsening after diagnosis: Significant worsening of symptoms in a COVID‑19 positive individual when imaging will modify management.

Post‑COVID impairment: Recovered COVID‑19 individuals with significant residual functional impairment and/or persistent hypoxemia, or concern for post‑COVID ILD where imaging will guide treatment.

Tuberculosis/NTM — CT Chest indications and follow-up

CT Chest (71250 or 71260) is covered when ANY of the following are met:

Positive screening with normal/equivocal x‑ray: Normal or equivocal chest x‑ray with newly positive PPD, other positive tuberculin skin test, or positive QuantiFERON‑TB Gold.

Suspected active TB: Suspected active or reactivated tuberculosis.

Complications/progression: Suspected complications or progression of tuberculosis (eg pleural tuberculosis, empyema, mediastinitis).

Suspected NTM‑PD: Suspected nontuberculous mycobacterial pulmonary disease.

Follow‑up frequency:

Fungal infections — CT Chest indications

CT Chest with contrast (71260) or HRCT without contrast (71250) is covered for:

Initial diagnosis of suspected fungal pneumonia or chest infection.

Complications/progression: Suspected complications or progression (worsening pneumonitis; pleural effusion, empyema, mediastinitis).

ABPA suspicion: Suspected Allergic Bronchopulmonary Aspergillosis (ABPA) in asthmatics with atypical presentation or poor response to conventional therapy.

Suspected sternal dehiscence — imaging

Imaging approach for suspected sternal wound dehiscence:

Initial assessment: Sternal wound dehiscence is primarily a clinical diagnosis; perform chest x‑ray prior to advanced imaging to assess sternal wire integrity and midsternal stripe.

CT for planning/diagnosis: CT Chest without contrast (71250) or with contrast (71260) is appropriate to differentiate wire migration from true sternal dehiscence and for preoperative planning or debridement/repair.

CT indicated when radiograph equivocal or for surgical planning.

Suspected sternal dehiscence

Imaging for suspected sternal dehiscence

Sternal dehiscence imaging: Chest x‑ray performed prior to advanced imaging; CT chest (71250 or 71260) appropriate to differentiate sternal wire migration from dehiscence and for preoperative planning when radiograph is equivocal.

Sarcoidosis imaging indications

Imaging for sarcoidosis

Establish or follow‑up sarcoidosis: CT chest (71250 or 71260) to establish or rule out sarcoidosis when suspected; in known sarcoidosis obtain CT for worsening or new symptoms or when treatment change is under consideration.

If CT equivocal, biopsy required for definitive diagnosis.

PET/CT for sarcoidosis: PET/CT (78815) is not for routine screening but can be considered to guide biopsy location, identify extrapulmonary biopsy sites when lung involvement is not apparent, differentiate reversible inflammation from fibrosis when it will affect treatment, or identify treatment failure where management will change.

Interpret PET findings in clinical context due to potential false positives.

Solitary pulmonary nodule imaging

Imaging and measurement guidance for solitary pulmonary nodules

Initial CT for discrete nodules: CT chest with or without contrast (71260 or 71250) is appropriate initially for discrete nodules seen on non‑dedicated imaging (eg chest x‑ray, CT abdomen, MRI spine, coronary CTA).

Dedicated combined without‑and‑with contrast (71270) and CTA (71275) are not considered initial dedicated studies in this context.

Measurement technique: Manual nodule measurements should use the average of long‑ and short‑axis diameters rounded to the nearest millimeter; a change is defined as an average diameter change ≥2 mm.>= 2 mm change

Largest nodule guides surveillance intervals; compare with earliest available and most recent prior CT.

Solid pulmonary nodules — single and multiple

Follow-up CT timing based on largest nodule size and number:

Solid single nodule <6 mm: Optional CT at 12 months; no routine further follow‑up if stable at 12 months (consider 12‑month CT in high‑risk or suspicious morphology).<6 mm

Solid single nodule 6‑8 mm: CT at 6‑12 months, then CT at 18‑24 months if stable; intervals depend on clinical risk.6-8 mm

Solid single nodule >8 mm: CT at 3 months, then CT at 6‑12 and 18‑24 months if stable; consider PET/CT (78815) or biopsy as indicated.>8 mm

Sub-solid pulmonary nodules (ground-glass and part-solid)

Follow-up intervals vary by size and presence of solid component:

Single pure GGO <6 mm: Consider follow‑up at 2 and 4 years; no routine follow‑up required for most patients.<6 mm

High‑risk patients may warrant imaging at 2 and 4 years.

Overall modality stance in the guideline is conservative: PET/MRI is generally not supported for routine use and should only be considered in limited circumstances when PET‑CT criteria are met and PET‑CT is unavailable; if used, report as CPT 78813 + CPT 76498.

Advanced MRI techniques that use inhaled hyperpolarized gases (for example Xenon Xe‑129) are treated as investigational/experimental and are not supported for routine coverage.

Post‑processing 3D rendering codes (CPT 76376/76377) have specified exclusions and must not be billed with the enumerated excluded modalities; routine 2D/3D projections derived from standard volumetric CT (e.g., MIP/MinIP) are included in base imaging codes and are not separately reimbursable.

For suspected bronchiectasis the guideline recommends HRCT (thin‑section CT) as the preferred modality; MRI chest is not recommended because it is inferior to CT for evaluation of lung parenchyma and is largely limited to research settings.

Note that CT without contrast (CPT 71250) is generally reserved for patients with severe renal dysfunction or life‑threatening iodinated contrast allergy when CT is required.

Routine advanced chest imaging is not supported for an acute COPD exacerbation; CT is rarely indicated for an uncomplicated exacerbation and should be reserved for cases where alternative diagnoses or pre‑procedural planning are under consideration.

Similarly, the guideline notes no literature supporting routine use of contrast‑enhanced CT for initial or follow‑up imaging of interstitial lung disease (ILD); IV contrast may be used selectively to evaluate alternate diagnoses involving the pleura, mediastinum, or pulmonary vasculature, but is not routinely required for ILD assessment.

CT chest is not indicated as a screening or first‑line diagnostic test for asymptomatic COVID‑19 or for individuals with only mild clinical features unless they are at elevated risk for disease progression or imaging would change management. Chest radiography remains the typical initial study.

Routine PET/CT screening for all individuals with sarcoidosis is not supported; PET/CT (CPT 78815) may be considered selectively—for example to guide biopsy site selection, detect extrapulmonary disease when no clear pulmonary target exists, or to differentiate active inflammation from fibrosis when the result would change management.

Serial PET studies without a new clinical indication are discouraged.

Maximum intensity projection (MIP) and minimum intensity projection (MinIP) are derived 2‑D projections from volumetric CT datasets and are considered part of the cross‑sectional imaging service. The guideline states these projections are included in the base imaging codes and separate reimbursement for MIP/MinIP is not supported.

The guideline indicates that PET/CT is not appropriate for evaluation of focal infiltrates, ground‑glass opacity, or isolated hilar enlargement. PET/CT sensitivity is limited for small or subsolid lesions and false positives can occur with inflammatory processes; PET/CT should be reserved for indications such as metabolic characterization of solid nodules (≥8 mm), staging, or when it will materially influence management.
