Bone Metastases — Palliative EBRT

General IGRT indications: IGRT is commonly indicated when target volume is located near or within critical structures, in tissue with inherent setup variation, when narrow margins are required to protect adjacent structures, when targets are subject to daily internal motion, when adjacent areas were previously irradiated and abutting fields must be precise, or when dose escalation is planned.

Intraoperative radiotherapy (IORT) used as accelerated partial breast irradiation (APBI) has been associated with higher ipsilateral breast recurrence in randomized trials and systematic reviews. The AHRQ review and individual trial data cited in this policy indicate IORT produces higher rates of ipsilateral breast recurrence compared with whole-breast irradiation; therefore IORT as PBI should be used cautiously and only in patients who meet modality- and patient-selection criteria supported by the evidence and guidelines.

Randomized trial evidence does not support escalation of conventionally fractionated chemoradiation to 74 Gy in 2 Gy fractions for unresectable stage III non–small cell lung cancer. The cited RTOG/NRG trial found worse overall survival and increased severe toxicity with 74 Gy versus standard 60 Gy with concurrent chemotherapy, and the policy therefore states that dose escalation to 74 Gy is not supported and may be harmful.
