Proven and Medically Necessary: Definitive Therapy primary site indications

IMRT is considered proven and medically necessary for definitive therapy to the primary site for any of the following conditions:

ANY of the following

Anus/anal canal cancer
Breast cancer when either: (1) the left-sided internal mammary nodes are being treated; OR (2) for accelerated partial breast irradiation of up to 5 fractions
Central nervous system (CNS) tumors (primary or benign) including brain, brainstem, and spinal cord
Cervical cancer
Endometrial cancer

Proven adjuncts

Proven adjunctive uses of IMRT and related advanced RT techniques (all criteria must be met where noted):

ALL of the following

Hippocampal-avoidance whole brain radiation therapy (HA-WBRT) of up to 10 fractions
- ALL of the following
  - Diagnosis of brain metastasis
  - Eastern Cooperative Oncology Group (ECOG) performance status ≤ 2 or Karnofsky performance status (KPS) ≥ 70
  - Prognosis of 4 months or greater

Proven and Medically Necessary: Brain metastasis with HA-WBRT criteria
