Clinical Recommendations & Evidence Summary

Guideline- and trial-derived recommendations summarized in the policy support several guideline-concordant regimens: for early breast cancer, hypofractionated whole-breast irradiation (HF-WBI) such as 40 Gy in 15 fractions (START) or 42.5 Gy in 16 fractions (Whelan) is safe and effective, and ultrahypofractionated five-fraction schedules (e.g., 26–30 Gy in 5 fractions) have demonstrated noninferiority in selected patients (FAST-Forward, Florence/APBI trials). Partial-breast/APBI regimens such as 30 Gy in 5 fractions are recommended by NCCN/ASTRO for selected patients, and boost dosing may be used for high local–recurrence risk.

For locally advanced stage III non–small cell lung cancer, definitive chemoradiation guideline and trial evidence support curative-intent dosing around 60 Gy delivered in 2 Gy daily fractions (~6 weeks), and dose escalation to 74 Gy was associated with worse outcomes in RTOG 0617.

For localized prostate cancer, multiple randomized trials and guidelines endorse moderate hypofractionation (for example 60 Gy in 20 fractions) as noninferior to conventional schedules; ultra-hypofractionation may be considered for selected patients, and whole-pelvic radiotherapy (WPRT) can improve biochemical failure–free survival in high-risk patients when nodal risk is ≥ 20% (POP-RT).

Evidence and guideline summaries support routine or frequent IGRT for sites with organ motion or tight margins (prostate, lung, head/neck, pelvis, complex abdominal sites), note that daily/frequent imaging reduces acute GI/GU toxicity and may improve biochemical control in prostate cancer, and that IGRT with respiratory management (including 4DCT/4D-IG/gating) can meaningfully reduce required margins. For left-sided breast RT, DIBH with image guidance is recommended to reduce heart dose and should be evaluated by DVH comparisons.

Breast Adenocarcinoma - EBRT Fractionation

Image-Guided Radiation Therapy (IGRT) - Medical Necessity

When the above criteria are not met, IGRT is not medically necessary including but not limited to: Superficial treatment of skin cancer including superficial radiation therapy or electronic brachytherapy; To align bony landmarks without implanted fiducials (e.g., during palliative radiation therapy)

Clinical recommendations: Professional society guidance recommends IGRT when high-precision localization is required (e.g., IMRT/VMAT, proton therapy, targets with motion, prior RT, fiducial-based localization); routine use for superficial skin treatments or simple bony alignment is not supported.

Operational note: For IGRT with 3D-CRT when justification is provided, include comparison plans and DVH data (if claiming DIBH benefit), documentation of immobilization intolerance, imaging demonstrating target motion, or rationale for reduced CTV margins. Examples of noncovered use include alignment to bony landmarks without fiducials and superficial skin treatments.

Denial Risk

IGRT not meeting criteria is not medically necessary

IGRT that does not meet the policy criteria is not medically necessary and may be denied. Examples include imaging used only for superficial skin treatments and use of imaging solely to align bony landmarks without implanted fiducials (for example, in palliative scenarios).

Superficial treatment of skin cancer (including superficial RT or electronic brachytherapy) where IGRT is not indicated.
Alignment to bony landmarks without implanted fiducials (e.g., during palliative radiation) is not sufficient justification for IGRT.

Billing Rule

Billing: IGRT, superficial RT, SBRT/SRS, and brachytherapy imaging

Do not report megavoltage treatment delivery CPT codes in conjunction with superficial radiation therapy (CPT 77401). IGRT should not be reported separately with SBRT or SRS. Imaging performed during brachytherapy should be reported with simple simulation code CPT 77280, not IGRT codes.

Do not report megavoltage delivery codes with CPT 77401; the following codes should not be reported with 77401 during the course of treatment: 77261, 77262, 77263, 77332, 77333, 77334, 77306, 77307, 77316, 77317, 77318, 77336, 77370, 77371, 77372, 77373, 77402, 77407, 77412, 77417, 0394T, 0395T, 77427, 77431, 77432, 77435, 77469, 77470, 77499.

Billing Rule

Billing: Special dosimetry and special physics services

Special dosimetry and complex physics services have specific single-reporting rules. Report CPT 77331 for special dosimetry measurements when the measurement is outside normal planning calculation parameters. Report CPT 77370 and CPT 77470 only once when the documented circumstances meet the listed complex-physics or procedural criteria.

CPT 77331: Use for special dosimetry (TLD, solid state probes, unusually small fields, abutting fields, selected brachytherapy situations, verifying dose under bolus). Frequency typically 1–6 measurements; additional requests evaluated case‑by‑case.
CPT 77370: Report once when indicated for special medical radiation physics consultation (e.g., complex photon/electron interactions, specialized brachytherapy, fusion of 3D image sets by a QMP in non‑IMRT plans).
CPT 77470: Report once for special treatment procedures (e.g., pediatric daily anesthesia, total/hemibody irradiation, combination EBRT and brachytherapy, cases very difficult to set up). Routine use is not appropriate; documentation of extra time/effort required.
CPT 77331, 77370, and 77470 are considered for coverage only when the primary radiation procedure is proven and medically necessary.
