Background & Evidence Summary

Bronchial thermoplasty is a minimally invasive bronchoscopic procedure that delivers controlled thermal radiofrequency (RF) energy to airway walls to reduce airway smooth muscle mass and responsiveness in adults with severe, persistent asthma not well controlled on medications. During the procedure a flexible bronchoscope is advanced and a specialized catheter applies controlled RF energy to targeted airway walls while aiming to spare surrounding tissue.

Treatment is administered across three outpatient visits about three weeks apart. Safety concerns include an increased risk of adverse events during the treatment period (procedural complications, emergency readmissions, overnight stays), with higher rates of peri-procedural AEs reported in some real-world registries compared with clinical trials; longer-term safety signals (eg, radiological changes) have been observed but clinical significance is uncertain.

The evidence base is limited and of low to moderate quality. Hayes concluded the overall body of evidence is low-quality and called for more RCTs and longer follow-up. ECRI found reported benefits to be modest and inconclusive and noted adverse events were more common with the device than sham. Systematic reviews and RCTs (eg, AIR/AIR2/RISA and subsequent reviews) report some modest improvements in quality of life and reductions in exacerbations in some trials, but no consistent improvement in lung function and limited long-term comparative data.
