Covered when ALL of the following are met

General upper extremity prosthesis criteria: Member has a traumatic or surgical amputation of the upper extremity or a congenital absence or defect; and prosthesis replaces all or part of the missing limb; and prosthesis will help the member regain or maintain function; and prosthetic device is ordered by or under the direction of a physician; and prosthetic needs evaluated for the member by a healthcare professional with appropriate prosthetic qualifications and training under the supervision of the ordering physician; and member is willing and able to participate in training for use of the prosthetic; and member with expected rehabilitation potential undergoes functional assessment including Activities of Daily Living (ADLs) and Instrumental ADLs (IADLs).

Pre-prosthetic assessment and prescription

Covered when ALL of the following are met

Pre-prosthetic evaluation and prescription: A comprehensive assessment by the care team documenting present health status, level of function, modifiable risk factors, pain, cognition/behavioral health, learning ability, residual limb and non‑amputated limb assessment, prosthetic goals, and vocational considerations; and a prescription specifying device design (preparatory vs definitive), control strategy (passive, externally powered, body‑powered, task specific), anatomical side and amputation level, socket interface and frame, suspension mechanism, and required terminal device, wrist, elbow, and/or shoulder components.

Based on VA/DoD pre‑prosthetic training and prescription recommendations; include team‑based decision rationale in documentation.

Bone-anchored percutaneous limb prostheses (for example, the OPRA Implant System) are considered unproven and not medically necessary based on the available evidence. The policy notes limited, low-quality studies and reports of high rates of infectious and mechanical complications that undermine conclusions about safety and efficacy, and therefore these devices are not covered as standard therapy.

Specifically, published series report modest implant survivorship with substantial complication rates (for example, retrospective reports with ≈83% survivorship at 5 years and a ~38% 5-year incidence of infectious complications at the skin-penetration site). Given the small cohort sizes, retrospective designs, and lack of comparative data to conventional socket prostheses, routine coverage is not supported and case-by-case review with prior authorization is advised when considered.

Use of osseointegrated (bone-anchored) arm prostheses in transhumeral (TH) amputations that result from vascular disease is not recommended in the cited literature. Available reports describe osseointegration primarily in transhumeral amputations from trauma or tumor, and the technique has not been tested in vascular-disease TH amputees.

Small trials and case series report benefit in some subgroups, but larger or comparative studies do not consistently demonstrate clear functional superiority over standard myoelectric hands.

Consequently, routine coverage of these advanced devices is not universally supported by the literature; the policy recommends careful patient selection, documentation of individualized need, training plans, and prior authorization for advanced myoelectric or osseointegrated solutions rather than blanket coverage.

Providers should verify whether a prior authorization (PA) is required for the specific HCPCS codes submitted and confirm any PA rules or edits that take effect on 11/01/2025.

Infectious and Implant Survival Risks — Document Safety Plan

Osseointegrated/percutaneous implants are associated with substantial infectious complication rates and variable implant survival; these safety concerns must be clearly addressed in the documentation supporting medical necessity.

Document history of inability to tolerate or severe problems with a conventional socket prosthesis (e.g., very short residual limb, severe socket-related skin breakdown).

Background and Context

Upper limb prostheses replace all or part of a missing arm or hand with the goal of restoring function for activities of daily living (ADLs), instrumental ADLs (IADLs), and return to work. Common amputation levels include transradial (below elbow) and transhumeral (above elbow), and device options range from passive cosmetic prostheses to body‑powered, externally powered/myoelectric, hybrid systems, and advanced neurointegrated or osseointegrated solutions.

Device selection should be individualized based on the level of amputation, remaining musculature and control signal availability, functional goals, rehabilitation potential, and risks associated with higher-technology options.
