The policy defines several device classes as unproven and not medically necessary. Specifically, bone-anchored percutaneous limb prostheses (e.g., the OPRA Implant System) are listed as unproven due to insufficient evidence of efficacy. Additionally, endoskeletal knee-shin systems with microprocessor control (swing/stance) are considered not medically necessary for individuals with MFCL K1 or K2 transfemoral amputations (including knee disarticulation), and for persons with hip disarticulation or hemipelvectomy. Finally, combined microprocessor-controlled ankle-foot systems with power assist are not medically necessary for transtibial, transfemoral (including knee disarticulation), or hip disarticulation/hemipelvectomy amputations. These exclusions reflect the policy determination that evidence is insufficient to support routine coverage for these device classes.

The OPRA (Osseoanchored Prostheses for the Rehabilitation of Amputees) Implant System is not supported for routine coverage because the clinical evidence is limited and of low quality and reports high rates of adverse outcomes. Systematic reviews and cohort studies identified improvements in mobility and patient-reported outcomes but also documented frequent and serious complications including high rates of infection (including osteomyelitis), substantial mechanical complications, and frequent revisions—some studies reporting revision rates above 85% in long-term follow-up. Given these safety concerns and the low quality of available studies, the policy concludes evidence is insufficient to establish the OPRA Implant System as effective and safe.

Powered microprocessor-controlled ankle-foot prostheses (powered MPAs) currently lack consistent, high-quality evidence of clinically meaningful benefit for transtibial amputees. An evolving evidence review (Hayes 2022, updated 2024) identified only a small number of poor- to very-poor-quality studies with inconsistent results and short follow-up; subsequent annual updates did not change the level of minimal support. Requests for powered MPAs therefore require strong individualized clinical justification because routine coverage is not supported by the available evidence.

FDA device information is provided for context only and is not a basis for coverage. The policy notes general FDA device classifications and examples but emphasizes that regulatory status alone does not establish medical necessity. For example, the OPRA Implant System is described in FDA material as an osseointegrated device that attaches a prosthesis to the femur, but approval or regulatory classification is informational and does not override the policy’s evidence-based coverage determinations.

Endoskeletal knee-shin systems that incorporate microprocessor control of both swing and stance phases are specifically limited by indication in this policy. They are considered unproven and not medically necessary for transfemoral amputees with lower functional levels (MFCL K1–K2), for knee disarticulation, hip disarticulation, or hemipelvectomy. In contrast, the policy recognizes that microprocessor knees have supportive literature for community ambulators; therefore, microprocessor knee coverage is contingent on functional level and meeting other clinical criteria.

The policy states that the evidence does not support use of microprocessor knees for patients with lower functional classification levels (K3). Microprocessor knees are generally supported in the literature for community ambulators (MFCL K3); however, their suitability for individuals at MFCL K0–K2 is not sufficiently demonstrated and these uses are considered not medically necessary.

For transtibial amputations specifically, the policy finds the evidence for powered MPAs to be limited and inconsistent. Hayes’ evolving evidence review (2022, updated 2024) reviewed a small number of poor-quality studies and concluded there is only minimal support for powered MPAs, with later updates not changing that conclusion. As a result, use of powered MPAs in transtibial amputees is not supported for routine coverage absent compelling supporting clinical data.

The policy recognizes that MPKs have substantial literature supporting benefit for community ambulators (MFCL K3), including reductions in falls and improvements in mobility in selected patients. By contrast, device classes such as bone-anchored percutaneous prostheses and powered MPAs have limited, low-quality or inconsistent evidence and are treated as unproven or not medically necessary in the indications described elsewhere in the policy.

Available studies of bone-anchored percutaneous limb prostheses (including long-term OPRA series) report improved prosthetic use and patient-reported outcomes in many participants, but these benefits have been accompanied by high rates of complications. Cohort reports and systematic assessments noted frequent superficial and deep infections, implant loosening, numerous mechanical complications, and substantial revision rates over long-term follow-up (10–18 years).
