Evidence-based coverage considerations

MPK benefits in K3+: Systematic reviews and multiple clinical studies support that MPKs improve safety (reduced falls), walking speed, and patient-reported mobility for community ambulators (K3).

Supported by Hahn et al. (2021) meta-analysis and additional trials; document MFCL and outcomes.

MPK for K2 — trial recommended: For limited community ambulators (K2), evidence suggests potential benefits (reduced falls, faster walking) but is limited; a documented trial fitting with functional outcome measures is recommended to establish individualized benefit before long-term authorization.

Powered ankle (MPA): There is insufficient evidence to support routine use of powered microprocessor prosthetic ankles (MPAs) for transtibial amputations; requests are likely to be considered not medically necessary without stronger evidence.

Hayes and other reviews conclude evidence is limited and small trials predominate.

The policy excludes certain advanced or implantable prosthetic technologies that lack sufficient evidence of safety and effectiveness. Specifically, bone-anchored percutaneous limb prostheses (e.g., OPRA Implant System) and specified microprocessor-controlled systems are considered unproven and not medically necessary for the listed indications. Coverage determinations for prosthetic devices continue to reference InterQual clinical criteria and applicable federal/state/contractual rules.

Use of the OPRA/osseointegrated implant systems is not supported by high-quality evidence and may be excluded or classified as not medically necessary. Published reviews and cohort studies report limited, low-quality data together with high rates of infection, mechanical complications, and revisions; therefore the device is considered unproven for transfemoral and related amputation levels.

Provision of powered microprocessor prosthetic ankles (MPAs) is not supported for routine use in transtibial amputations because current clinical evidence is insufficient to establish consistent clinical benefit. Requests for powered MPAs should be evaluated against existing evidence and may be denied when documentation does not demonstrate medically necessary benefit.

Available studies for powered MPAs are small, of variable and generally low quality, and show inconsistent results for outcomes such as walking speed, metabolic cost, and activity. Systematic reviews and evolving evidence reviews (e.g., Hayes) conclude there is insufficient evidence to support routine clinical use of powered MPAs for transtibial amputees.

Bone-anchored percutaneous limb prostheses (including OPRA and similar osseointegrated systems) and other advanced implantable prosthetic approaches are specifically identified as excluded or investigational when evidence does not demonstrate acceptable safety or effectiveness. High complication and revision rates reported in longitudinal studies and reviews inform this exclusion.

The policy classifies OPRA and related osseointegrated systems as investigational or not medically necessary when the medical literature shows limited, low-quality evidence and substantial risks. Studies report improved patient-reported outcomes in some cohorts but consistently show high rates of infections, mechanical failures, and frequent revisions; these safety concerns and uncertain long-term net health benefit support the investigational/not medically necessary classification.

Based on current evidence, the policy considers provision of powered MPAs for transtibial amputees not medically necessary because demonstrated clinical benefits are inconsistent and insufficient to justify routine coverage.

Overall, powered microprocessor prosthetic ankles have minimal and inconsistent support in the clinical literature. Randomized trials and small comparative studies have limitations (small sample sizes, short follow-up, device weight/battery issues) and do not provide generalizable evidence of consistent functional advantages over unpowered prostheses.

Requests should reference InterQual® Durable Medical Equipment, Prosthetics, Lower Extremity criteria.

Trial Fitting and Functional Evidence Recommended

Trial fitting and objective functional evidence are recommended to support the medical necessity of advanced prosthetic components (for example, microprocessor knees or powered ankles). Trial fitting results demonstrating improved function, reduced falls/stumbles, increased walking speed, or other measurable benefit at the patient’s Medicare Functional Classification Level (MFCL/K-level) strengthen the authorization request.

Report trial duration, assessment tools used (e.g., L-Test, 6-minute walk, step counts), and pre- and post-trial outcome measures.
