The policy also designates endoskeletal knee-shin systems with microprocessor control as unproven and not medically necessary for amputees with K1 or K2 classifications in the specified anatomic levels.

Per the policy history and Application section, this document was updated to explicitly note that it does not apply to Idaho and Kansas; providers should consult the Idaho- and Kansas-specific Lower Extremity Prosthetics policy/guideline for coverage rules and requirements in those states.
