Observation services are considered medically necessary when the following conditions apply:

Observation medical necessity: Member requires at least one of: short-term monitoring expected to require >=6 hours of assessment or treatment and improve significantly within 24-48 hours; acute treatment and reassessment; or diagnostic evaluation to establish a treatment plan (including event monitoring or response to therapy that may require immediate intervention).>=6 hours; improvement in 24-48 hours
