Examples of services and items that are excluded from coverage include elective non-medically necessary surgeries and procedures, experimental or investigational procedures, items, and treatments, and personal or comfort items. Prescription medications are generally not covered under the hospital benefit and are addressed under the member's pharmacy benefit. Other non-covered items include private duty nursing care, private rooms unless medically necessary, and any services or items that are not medically necessary for the diagnosis, care, or treatment of an illness or injury.

Authorization is required for non-emergent inpatient and certain outpatient hospital services. Prior authorization must be obtained from the member's network medical group or UnitedHealthcare for inpatient admissions and for outpatient services when specified by the plan. Emergency or urgently needed services are exceptions and may be covered without prior authorization (see Emergency and Urgent Services policy).

Non-medically necessary, experimental/investigational, or non-authorized services are subject to denial. Outpatient surgeries or procedures that are not medically necessary or that lack required authorization may be non-covered and denied.
