Coverage Criteria — Observation Services

inv-01: Medically necessary observation services

Observation services are considered medically necessary when ANY of the following are met:

Observation medical necessity: At least one of the following: short-term monitoring for a condition expected to require at least 6 hours of assessment or treatment and improve significantly within 24-48 hours; acute treatment and reassessment; or diagnostic evaluation to establish a treatment plan (including event monitoring or response to therapy that may require immediate intervention).see node text

inv-02: Not medically necessary / exclusions

Observation services are NOT medically necessary when:

Non-medical reasons: Provided for the convenience of the hospital, physicians, members, or members' families, or while awaiting placement to another health care facility.

Claims may be denied if these conditions apply.

Obstetric exclusion: Observation services portion of this policy does not apply to obstetric members during pregnancy, childbirth, or the post-partum period.

Separate obstetric coverage applies.

Observation services are not medically necessary when provided solely for non-clinical reasons. Examples include care delivered for the convenience of the hospital, physicians, members, or the member’s family. Claims may be denied when observation is used for these administrative or convenience purposes rather than for an appropriate clinical indication.

The observation services portion of this policy does not apply to obstetric members during pregnancy, childbirth, or the post‑partum period. Obstetric care follows separate coverage determinations; do not use this observation policy to determine level of care for maternity‑related admissions.

Use of observation solely to await placement to another health care facility or for other placement-related delays is not medically necessary. Observation should not be used as a holding status for bed placement or disposition logistics; clinical indications must justify the hospital‑level monitoring or treatment.
