Defines UnitedHealthcare coverage guidance for observation and inpatient hospital services for members in Idaho (including Idaho Medicaid Plus). It governs level-of-care decisions and use of InterQual criteria for clinical assessment.
Removed language indicating observation services are considered medically necessary for a member who requires short-term monitoring of at least 6 hours with expected improvement within 24-48 hours and at least one listed condition (acute treatment and reassessment; event monitoring; diagnostic evaluation).

Observation level of care is often used for the following conditions (not all-inclusive):

Observation services are considered medically necessary only when used to provide short-term monitoring, treatment, reassessment, event monitoring, or diagnostic evaluation for an acute condition where those services cannot be safely delivered in a lower level of care. Observation services are not medically necessary for the convenience of the hospital, physicians, individuals, or the individual's family, or while awaiting placement to another health care facility.
The observation services portion of this policy does not apply to an obstetric member during pregnancy, childbirth, or the post‑partum period. For obstetric care related to pregnancy, labor, delivery, and immediate post‑partum management, follow the applicable maternity and obstetric benefit policies and coverage rules instead of this observation policy.
