Evidence summaries: Retrospective cohort studies and systematic reviews report mixed outcomes for concurrent panniculectomy. Selected studies: Gebran et al. (2021) found no significant differences in 30‑day mortality, wound complications (11.5%), readmission, or reoperation when panniculectomy was performed at the time of bariatric surgery in selected patients but noted higher wound complication risk with BMI >50 and prolonged hospital stay; Nag et al. (2021) found increased superficial and wound infections, pulmonary embolism, sepsis, return to OR, and longer operative time/LOS for panniculectomy combined with gynecologic surgery in obese patients and no consistent benefit in comparative studies; Prodromidou et al. (2020) reported comparable intra‑ and post‑operative complications for panniculectomy combined with endometrial cancer surgery versus conventional laparotomy and lower wound breakdown in the combined group but cautioned interpretation due to limited retrospective studies; Sosin et al. (2020) meta‑analysis of simultaneous ventral hernia repair and panniculectomy (917 patients) reported a mean surgical‑site occurrence of 27.9% and hernia recurrence of 4.9% with most common complications being superficial SSI (15.8%) and seroma (11.2%).

Guideline summaries: ASPS (2017) states panniculectomy may be considered a functional correction for patients of appropriate height/weight with persistent panniculitis, chronic back pain, or functional impairment after adequate non‑surgical management, and notes higher BMI is associated with increased surgical complications. The SOGC guideline (Yong et al., 2019) indicates panniculectomy can be considered at the time of open hysterectomy in patients with obesity but that existing studies are small and primarily retrospective; when combined procedures are performed, postoperative antibiotics should be considered.

UnitedHealthcare excludes cosmetic procedures from coverage. Examples of excluded body contouring procedures include, but are not limited to, abdominoplasty, lipectomy (including suction-assisted lipectomy unless part of an approved procedure), and repair of diastasis recti. Refer to the member-specific benefit plan document for applicable state or plan exceptions (some states may mandate coverage for procedures otherwise considered cosmetic).

Prior policy language explicitly labeled several body contouring procedures as cosmetic and not medically necessary (for example, abdominoplasty; lipectomy including suction-assisted lipectomy unless part of an approved procedure; and repair of diastasis recti). That language has been removed in this revision of the policy; the policy title was changed from "Panniculectomy and Body Contouring Procedures" to "Panniculectomy" and the prior exclusion text was deleted from the Coverage Rationale and Definitions sections.

There is insufficient quality evidence to conclude that performing panniculectomy concurrently with abdominal or gynecologic surgery (including but not limited to hernia repair, bariatric surgery, Cesarean section, or hysterectomy) provides net benefit that outweighs the potential for increased complications. Additional peer-reviewed literature is needed to determine long-term benefits and the impact on surgical complications and recovery when panniculectomy is performed at the same operative setting.

This portion of the policy does not include new explicit "not medically necessary" determinations for specific body contouring procedures; prior exclusions for certain procedures were removed during the policy revision. Clinical Background

An abdominal panniculus is an overhanging apron of redundant skin and subcutaneous fat that commonly occurs after massive weight loss. It can lead to recurrent skin problems such as intertrigo, chronic rashes, pustules, and localized infections, and may interfere with hygiene and activities of daily living. Panniculectomy removes this excess skin and subcutaneous tissue (typically without abdominal muscle plication) to address these functional problems.
