When panniculectomy is performed for primarily cosmetic purposes or for any condition that does not meet the InterQual criteria, it is considered not medically necessary. The policy also states panniculectomy performed in conjunction with abdominal or gynecologic surgery (for example, hernia repair, bariatric surgery, Cesarean section, or hysterectomy) is not covered unless the InterQual CP criteria are satisfied. Clinical judgment and InterQual criteria determine whether concurrent procedures are acceptable and whether the panniculectomy portion is reconstructive.

Providers must obtain prior authorization when coverage is being evaluated against the InterQual CP; certain CPT codes may be considered either cosmetic or reconstructive and will require review. Verify member benefit plan and state/contractual requirements before scheduling combined procedures, and ensure documentation addresses functional impairment, prior conservative management, and the anatomic extent of the panniculus per InterQual guidance.

During the November 1, 2025 revision the policy title was narrowed from "Panniculectomy and Body Contouring Procedures (for Nebraska Only)" to "Panniculectomy Surgery (for Nebraska Only)." As part of this operational update, prior language that explicitly labeled a set of body contouring procedures (for example, abdominoplasty, lipectomy including suction‑assisted lipectomy, and repair of diastasis recti) as cosmetic and not medically necessary was removed from the policy text and definitions. Background

An abdominal panniculus is an overhanging apron of skin and subcutaneous fat that commonly follows massive weight loss. This excess tissue can cause recurrent skin irritation, panniculitis, or superficial infections, intertrigo or rashes in the dependent folds, and difficulty with hygiene and activities of daily living. Panniculectomy removes the hanging skin and subcutaneous tissue (without muscle plication) to address these functional problems when clinical criteria are met.
