Covered when ALL of the following are met:

Lipedema diagnosis and findings: Diagnosis of lipedema with: absence of pitting edema; bilateral and symmetrical manifestation with minimal involvement of the feet; disproportionate adipocyte hypertrophy of the affected extremity; photographs documenting disproportional fat distribution consistent with diagnosis; failure of the limb adipose hypertrophy to respond to recommended bariatric surgery or other medically supervised weight loss modalities if Class II or III Obesity; negative Stemmer sign; pressure-induced pain and tenderness on palpation.

Conservative treatment trial: Failure to respond to three or more months of conservative treatment (compression or manual therapy).

Treatment planning and approvals: Assessment by the referring primary care provider or a vascular specialist (different from the treating surgeon) confirming lipedema is an independent cause of the functional impairment and that surgery is expected to restore or improve function; documentation that liposuction for the extremity or trunk in its entirety will take place within a 12-month period following the initial surgical treatment (unless medically contraindicated); when more than one procedure is necessary on the same region, documentation that the liposuction volume exceeds a clinically acceptable amount for one surgery (more than 5000 cc total aspirate); and a postoperative plan to continue wearing compression garments and continue conservative treatment.

The National Institute for Health and Care Excellence (NICE) concluded in March 2022 that the evidence on the safety and efficacy of liposuction for chronic lipedema is inadequate, citing concerns about major adverse events (for example, fluid imbalance, fat embolism, deep vein thrombosis, and local anesthetic toxicity) and methodological limitations of available retrospective studies. NICE therefore recommends that liposuction for chronic lipedema be used only in the context of research, a stance that informs but does not automatically determine coverage under other plan rules.

For emphasis, procedures performed primarily for cosmetic purposes or liposuction of a trunk or extremity that was previously fully treated are not covered as reconstructive or medically necessary under this policy. Such scenarios are explicit triggers for denial unless the documentation demonstrates a separate, new functional impairment meeting coverage criteria.

Where a local contract or plan adopts NICE-style language or explicit research-only restrictions, liposuction for lipedema performed outside a documented research context may be considered not medically necessary per those contractual rules.
