Medically Necessary — Liposuction for Lipedema

Liposuction is reconstructive and medically necessary to treat a Functional Impairment when ALL of the following are met:

ALL of the following

Diagnosis of Lipedema requires ALL of:
- Absence of pitting edema
- Bilateral and symmetrical manifestation with minimal involvement of the feet
- Disproportionate adipocyte hypertrophy of the affected extremity
- Photographs of the area to be treated that document disproportional fat distribution consistent with diagnosis
- Failure of the limb adipose hypertrophy to respond to recommended bariatric surgery or other medically supervised weight loss modalities, if Class II or III Obesity
  Applies only when member meets Class II/III obesity definitions
- Negative Stemmer Sign
- Pressure-induced pain and tenderness on palpation
Failure to respond to three or more months of Conservative Treatment (compression or manual therapy)
Treatment plan must include ALL of:
- Assessment by the referring primary care provider or a specialist in vascular conditions (different from the treating surgeon) confirming that Lipedema is an independent cause of the Functional Impairment and that surgery is expected to restore or improve the Functional Impairment
  Referring assessor must be different from treating surgeon
- Documentation that liposuction for the extremity or trunk in its entirety will take place within a 12-month period following the initial surgical treatment (unless medically contraindicated)
- When more than one procedure is necessary on the same region of the extremity and/or trunk, documentation that the liposuction volume exceeds a clinically acceptable amount for one surgery (more than 5,000 cc total aspirate)
- Postoperative plan of care to continue wearing compression garments as instructed and to continue Conservative Treatment

Not Medically Necessary / Not Reconstructive

Liposuction for Lipedema is not reconstructive and/or medically necessary in the following situations:

ANY of the following

When performed for cosmetic purposes (procedures/services that change or improve appearance without significantly improving Functional Impairment)
When performed on the trunk and/or extremity that was previously treated in its entirety

Include clear photographs of the area to be treated that document disproportional fat distribution and chart clinical findings showing absence of pitting edema, a negative Stemmer sign, bilateral and symmetrical involvement (with minimal foot involvement), pressure‑induced pain/tenderness on palpation, and disproportionate adipocyte hypertrophy.

Document failure to respond to three or more months of Conservative Treatment—such as compression therapy or manual therapy—prior to approval for liposuction.

When more than one procedure is planned for the same region, document that the total aspirate volume exceeds a clinically acceptable amount for a single surgery (more than 5,000 cc) to justify serial or large‑volume procedures and document that planned liposuction for the extremity or trunk will be completed within 12 months; applicable CPT codes: 15877, 15878, 15879.

Document the postoperative plan of care specifying continued use of compression garments as instructed and the continuation of Conservative Treatment (e.g., compression and manual therapy).

Clinical and Policy Context

Lipedema is a chronic, progressive adipose disorder that primarily affects women and is characterized by disproportionate subcutaneous fat deposition of the lower and/or upper extremities and sometimes the trunk, often with bilateral symmetry, pain on pressure, bruising, minimal pitting edema and a negative Stemmer sign. Conservative management is considered first-line and includes a healthy lifestyle (diet and exercise), complete decongestive therapy (bandaging, compression garments, manual lymphatic drainage), and psychosocial support; these measures aim to minimize symptoms, prevent progression, and improve function. When conservative measures fail, surgical removal of abnormal lipedema tissue by liposuction can be considered to improve pain, function, swelling, and quality of life.

Functional or Physical or Physiological ImpairmentDeviation from normal function causing significant limitation in movement or activities of daily living, evidenced by difficulty in physical/motor tasks or basic life functions.

LipedemaAn adipose tissue disorder with disproportionate subcutaneous fat on buttocks, hips, lower (and sometimes upper) extremities; often bilateral/symmetrical, painful to pressure, negative Stemmer sign, and may fail to respond to extreme weight loss.

Stemmer SignPhysical test for lymphedema: inability to pinch the skin on the dorsum of the foot or hand is a positive sign; policy requires a negative Stemmer Sign for lipedema diagnosis.
