Documentation of diagnostic criteria and prior conservative therapy

Document the diagnostic criteria for lipedema: absence of pitting edema, bilateral and symmetrical distribution with minimal foot involvement, disproportionate adipocyte hypertrophy, and a negative Stemmer sign. Include photographs of the area to be treated that demonstrate disproportional fat distribution. Document failure of at least 3 months of conservative treatment (e.g., compression or manual therapy). If the patient has Class II or III obesity, document failure of bariatric surgery or other medically supervised weight loss modalities when applicable.

Background and Evidence Summary

Background: Lipedema is a chronic, progressive adipose disorder often misdiagnosed as lymphedema or obesity and most commonly affects women with disproportionate subcutaneous fat on the buttocks, hips, and extremities. Conservative treatments include lifestyle measures (diet and exercise) and complete decongestive therapy (bandaging, compression garments, manual lymphatic drainage) and other supportive care. Liposuction (commonly including tumescent anesthesia (TA) liposuction, water-assisted liposuction (WAL), and power-assisted liposuction (PAL)) may reduce pain, edema, bruising and improve mobility and quality of life. Evidence and guideline summaries note low complication rates but variable quality of studies, and that multiple procedures (serial/staged treatments) are commonly required depending on volume to be removed; postoperative continuation of conservative therapy and use of compression garments is generally recommended. Techniques aim to be lymph-sparing and choice of approach should minimize trauma to vessels, nerves, and lymphatics.

Large volume liposuction site-of-service considerations

For large-volume liposuction (greater than 5,000 cc total aspirate), follow guidance that the procedure be performed in an acute-care hospital or an accredited/licensed facility and consider performing large-volume procedures as separate serial procedures when appropriate. Document facility accreditation or acute-care hospital setting and perioperative risk considerations per ASPS/ specialty guidance.
