Coverage Criteria for Mastectomy to Treat Gynecomastia

Reconstructive / medically necessary mastectomy criteria

Covered when ALL of the following are met

Eligibility: Member is male aged 18 or over.all

Clinical severity and symptoms: Gynecomastia stage II, III, or IV with moderate to severe chest pain causing a Functional or Physical Impairment (inability to participate in athletic events, sports, or social activities alone does not qualify as a Functional or Physical or physiological Impairment).all

Tissue characterization: Glandular breast tissue is the primary cause of gynecomastia (pseudogynecomastia—fatty enlargement—is excluded) and is documented on physical examination and/or mammography.all

Persistence after eliminating reversible causes: Gynecomastia persists after cessation of prescribed medications, nutritional supplements, and appropriate screening(s) for non-prescription and/or recreational substances known to cause gynecomastia (examples include but are not limited to testosterone, marijuana, asthma drugs, phenothiazines, anabolic steroids, cimetidine, and calcium channel blockers).all

Appropriate medical evaluation: Appropriate evaluation of medical causes has been performed with normal supporting laboratory testing. Supporting tests may include but are not limited to: hormone testing (beta-hCG, thyroid studies, sex hormone binding globulin, estradiol, FSH, LH, prolactin, testosterone), liver enzymes, serum creatinine, and alpha-fetoprotein.all

If a tumor or neoplasm is suspected regardless of age, perform breast ultrasound and/or mammogram and manage as indicated.

Pseudogynecomastia characterized by predominant fatty enlargement rather than glandular breast tissue is excluded from coverage for mastectomy unless exam or imaging documents that glandular tissue is the primary cause of the breast enlargement.

Gynecomastia that is solely cosmetic — without documented Functional or Physical Impairment (for example, inability to participate in sports or social activity alone does not qualify) is not covered as reconstructive or medically necessary.

Conservative Management and Evaluation Required

Conservative management and evaluation of underlying causes are expected prior to surgery. This includes cessation and evaluation of medications, supplements, or substances known to cause gynecomastia, appropriate laboratory testing (e.g., hormone panel, liver enzymes, AFP, creatinine), and specialist workup when indicated. Surgical treatment is intended for persistent gynecomastia that does not resolve after addressing reversible causes.

Background

Gynecomastia is a benign enlargement of male breast tissue caused by an increase in glandular tissue. Physiologic forms occur in newborns, adolescents, and older men, while nonphysiologic causes include systemic disease, medications, supplements, or tumors. Surgical options for persistent, symptomatic, or severe gynecomastia include open or minimally invasive mastectomy techniques and liposuction-assisted approaches. The American Society of Plastic Surgeons classifies gynecomastia severity as Grade II (moderate enlargement exceeding the areola), Grade III (moderate enlargement with skin redundancy), and Grade IV (marked enlargement with skin redundancy and feminization), which are the grades addressed by the coverage criteria.
