Coverage Criteria for Breast Reconstruction

Reconstructive indications

Covered when reconstruction is for restoration after oncologic resection, congenital anomaly, or severe disfigurement

Primary indications: Breast reconstruction during or after mastectomy, or to restore appearance after lumpectomy for breast cancer, reconstruction for congenital disorders (e.g., Poland syndrome, Turner syndrome), or severe breast disfigurement (e.g., amastia, radiation- or trauma-related deformity) is considered reconstructive and medically necessary in appropriate circumstances.

Cosmetic exclusions

Excluded/Not covered cosmetic indications

Cosmetic exclusions: Breast reconstruction for asymmetry and procedures that correct an anatomical congenital anomaly without improving or restoring physiologic function are considered cosmetic and not medically necessary. Specific exclusions (see Benefit Considerations) include aberrant breast tissue, aspirations, biopsy, duct lesions, excision of cysts, fibroadenomas, nipple or areolar lesions, treatment of gynecomastia, revision for normal aging, tissue protruding at end of scar ('dog ear'), and cosmetic procedures not related to mastectomy such as breast enhancement (e.g., implants, mastopexy), liposuction, and breast surgery solely to create symmetry.

Requests for procedures listed as cosmetic may be denied as not medically necessary.

Implant removal and nipple repair

Specific procedural coverage related to implants and nipple procedures

Implant removal and nipple repair: Removal of breast implants is considered reconstructive and medically necessary for individuals implanted with Allergan BIOCELL textured implants regardless of the reason for initial placement; implant removal may be performed with or without capsulectomy/capsulotomy per clinical indication (see InterQual CP: Procedures, Breast Implant Removal). Nipple/areolar reconstruction (including tattooing) and correction of inverted nipples are considered reconstructive when documented criteria are met (e.g., chronic nipple discharge, bleeding, scabbing, ductal infection, or when resulting from a congenital anomaly).

Reconstructive adjuncts

Adjunct procedures considered reconstructive when performed with reconstruction

Adjuncts: Procedures performed in conjunction with breast reconstruction that may be considered reconstructive and medically necessary include creation of the nipple and areola (including tattooing), mastopexy or breast reduction if required prior to mastectomy to preserve nipple viability, implantation of tissue expanders as an initial phase of reconstruction, and use of acellular dermal matrix (ADM) products (e.g., AlloDerm, Cortiva AlloMax, DermACELL, FlexHD).

ADM use is listed as implantation of biologic implant (CPT 15777); tattooing codes (11920–11922) and tissue expander/implant codes (11970–11971, 19340, 19342) are in applicable codes.

Post-mastectomy complications

Treatment of complications post-mastectomy

Complications covered: Treatment of post-mastectomy complications is considered medically necessary and includes management of lymphedema (including complex decongestive physiotherapy), post-operative infections, and supportive devices such as lymphedema pumps (considered durable medical equipment) and compression lymphedema sleeves (considered prosthetic devices).

Elastic bandages and wraps associated with medically necessary lymphedema treatment are included.

Reconstructive breast surgery is intended to restore breast appearance and, when applicable, physiologic form after lumpectomy or mastectomy for cancer. Reconstruction may be performed immediately at the time of mastectomy (same‑day implant insertion) or delayed until after completion of adjuvant therapy. Indications also include congenital disorders (e.g., Poland syndrome, Turner syndrome), severe disfigurement from radiation or trauma, and removal of implants when clinically indicated. Procedures that correct an anatomical congenital anomaly but do not improve or restore physiologic function are considered cosmetic and are excluded from coverage. When an intervention for a congenital breast anomaly does not restore function, it is not considered reconstructive under this policy and therefore is not covered.

Breast reconstruction performed solely to address asymmetry or for other primarily cosmetic indications is not medically necessary. Requests for reconstruction for asymmetry or similar cosmetic reasons may be denied; providers should refer to the Benefit Considerations for examples of cosmetic procedures.

Breast reconstruction procedures undertaken to correct cosmetic abnormalities that do not restore physiologic function are excluded from coverage. Examples listed in Benefit Considerations include breast enhancement (e.g., implants, mastopexy), liposuction, surgery performed solely to create symmetry when not related to mastectomy, revision for normal aging, and correction of minor scar contour issues such as a tissue protruding at the end of a scar ('dog ear').
