The independent HTA by Hayes found insufficient high-quality evidence to support mechanized spinal distraction therapy and assigned a Hayes rating of D (insufficient evidence) for multiple low back pain–related diagnoses. Hayes also identified specific populations with potential safety concerns (for example, pregnancy, unstable spine, spinal surgical implants, severe osteoporosis, and cauda equina syndrome) for which evidence is particularly lacking.

Use of motorized spinal traction (also referred to as vertebral axial decompression) is explicitly classified as not medically necessary for neck and low back disorders. Claims for these services may be denied on the basis that the devices and therapies are unproven and lack sufficient evidence of efficacy.
