Coverage Criteria

General Habilitative Coverage Criteria

Provider and setting requirements: Ordered or performed by a physician and administered by a licensed therapy provider, qualified autism service provider, or other provider acting within the scope of their license or as authorized under California law; provided in a physician's office or on an outpatient basis at a hospital or alternate outpatient rehabilitative facility.

Medical necessity: Services are medically necessary to treat a disabling condition and documentation supports the need for initial or continued treatment with progression toward treatment plan goals.

Services are not covered when documentation does not support medical necessity due to inability to progress toward goals or when treatment goals have been met.

Covered service types: Service types include outpatient physical therapy, occupational therapy, speech therapy, post-cochlear implant aural therapy, cognitive habilitative therapy, and manipulative treatment as applicable to the member's disabling condition.

Refer to member's EOC/SOB for plan-specific benefit applicability (for Essential Health Benefit plans, habilitative services are covered for members with disabling conditions including autism spectrum disorder).

Benefit plan provisions and site-of-care exceptions: Coverage is subject to the member's Evidence of Coverage/Schedule of Benefits; services received inpatient are covered only when the member is admitted or the services are authorized by the member's network medical group or UnitedHealthcare.

The policy excludes services that are not habilitative in nature or that serve recreational, social, or general fitness purposes. Examples include activities and memberships such as gym and fitness club memberships and fees, health club fees, dancing classes, exercise equipment or supplies, and similar programs. Biofeedback services are excluded on most plans — always check the member's Evidence of Coverage/Schedule of Benefits. Cardiac and pulmonary therapy are also not considered habilitative services under this policy. Additional non-covered categories include custodial care, respite care, day care, therapeutic recreation, vocational training, residential treatment, hypnotherapy, programs that do not require physician or licensed therapy supervision, and work hardening.

Speech, occupational, or physical therapy services are not covered when documentation does not support medical necessity. This includes situations where records demonstrate the member is unable to progress toward treatment plan goals or where treatment goals have already been met. Services may also be denied when they exceed any visit limits specified in the member's Evidence of Coverage/Schedule of Benefits, are considered unproven/investigational, or do not help the member meet or maintain functional goals within a prescribed timeframe.

Habilitative services received while in an inpatient setting (e.g., inpatient hospital, inpatient rehabilitation facility, or skilled nursing facility) are covered only when the member is admitted or the services are authorized by the member's network medical group or UnitedHealthcare. Refer to the member's Evidence of Coverage (EOC) Schedule of Benefits (SOB) for details.

Inpatient habilitative services require admission or prior authorization by the member's network medical group or UnitedHealthcare.
Check the member's EOC/SOB for any plan-specific inpatient coverage rules and limits.

Provider actions

Providers must ensure orders and services are consistent with the scope of their license or certification under California law. Therapy orders should be signed by a physician and services administered by a licensed therapy provider, qualified autism service provider, or other authorized provider.

Verify provider licensure/certification before billing.
Ensure physician orders are on file for habilitative therapy services.

We may require documentation to substantiate that initial or continued habilitative treatment is needed. Maintain and submit supporting clinical documentation consistent with UnitedHealthcare medical policy requirements.

Documentation may include treatment plans, progress toward goals, therapy notes, and physician orders.

Services may be denied when documentation does not support medical necessity. Common denial risks include lack of progress toward treatment goals, treatment already completed, services beyond visit limits in the member's EOC/SOB, services considered investigational/unproven, or services that are custodial, recreational, or educational in nature.

Background

Habilitative services are intended to help members with disabling conditions acquire, maintain, or improve skills and functioning, including services for conditions such as autism spectrum disorders. To be covered, services must be medically necessary, ordered by a physician, and provided by appropriately licensed therapy providers or qualified autism service providers acting within the scope of their license. Covered services are generally delivered in outpatient settings or a physician's office; inpatient habilitative services may be covered when the member is admitted or authorized. Coverage is subject to the member's Evidence of Coverage/Schedule of Benefits and applicable state mandates.

Work Hardening/Work RehabilitationAn interdisciplinary program consisting of physical therapy, occupational therapy and counseling professionals that provides structured treatment to progressively improve physical function as a transition between acute care and return to work.
