Coverage & Medical Necessity Criteria

Medical necessity per Nebraska DHHS

Coverage and medical necessity are determined by adherence to Nebraska DHHS rules:

State DHHS criteria required: Services are covered when they meet the medical necessity clinical coverage criteria specified by Nebraska Department of Health and Human Services (471 NAC Chapters 14, 18, and 23).

Provider must follow referenced DHHS chapters for clinical criteria.

The code S8990 is included to identify services described as physical or manipulative therapy performed for maintenance rather than restoration. Therapies provided primarily for maintenance — i.e., to preserve current function without expectation of restoring lost function — may not meet restoration-based medical necessity criteria. Inclusion of this code in the list does not guarantee coverage; providers must document how the therapy meets the applicable clinical medical necessity criteria under the member's governing benefit rules.
