Medically Necessary Indications (Overload Conditions)

Chelation is considered proven and medically necessary for documented heavy metal toxicity and overload conditions.

Primary coverage condition: Documented heavy metal toxicity or overload condition (e.g., iron, copper, lead, aluminum) established by appropriate diagnostic workup.

Diagnostic workup must consider the individual's history and validated testing in appropriate biological samples.

Not Medically Necessary / Unproven Indications

The following uses are considered unproven and not medically necessary due to insufficient evidence:

Mercury from dental amalgam: Chelation therapy for mercury 'toxicity' attributed to dental amalgam fillings.

High-quality trials and reviews do not support clinical harm from amalgams and do not justify chelation.

Chronic non-overload diseases: Chelation therapy for treating chronic, progressive non-overload diseases (including cardiovascular disease, diabetes, cancer, Alzheimer's disease, autism spectrum disorder, Parkinson's disease, and other listed conditions).

Well-designed peer-reviewed studies do not support chelation for these conditions; evidence is insufficient for safety and efficacy.

Medical necessity for chelation

Covered only when ALL of the following are met:

Documented metal intoxication: Confirmed diagnosis of metal intoxication or overload using validated testing in appropriate biological samples.

AAFP and ACMT state chelation is appropriate only for documented intoxication diagnosed with validated tests.

Appropriate agent and indication: Treatment uses a chelating agent and regimen consistent with accepted toxicology practice or FDA-approved indications for that agent (for example, specific iron chelators for iron overload or agents indicated for lead poisoning).

Professional guidance notes certain chelators are approved for specific overload indications; EDTA is not approved for CVD prevention/treatment.

Not medically necessary / investigational uses

Not covered / Not medically necessary when ANY of the following apply:

Neurodegenerative and neurodevelopmental disorders: Chelation provided for Alzheimer's disease, autism spectrum disorder, Parkinson's disease, or similar neurodegenerative/ neurodevelopmental conditions.

Existing studies are small, low quality, or show no benefit; safety concerns have been reported (e.g., hypocalcemia, renal impairment, death).

Cardiovascular disease or prevention post-MI: Chelation therapy used to prevent or treat coronary artery disease, stable ischemic heart disease, or to reduce cardiovascular events post-MI.

RCTs and systematic reviews do not demonstrate consistent clinical benefit; major cardiology societies and guidelines recommend against routine use.

Mercury from dental amalgam fillings: Chelation for presumed toxicity from dental amalgam fillings.

Randomized trials and systematic reviews show no evidence that amalgams cause clinical harm sufficient to justify chelation.

Chelation therapy is excluded and considered not medically necessary for chelation claims related to non-overload conditions, including attempts to treat chronic, progressive diseases where heavy metal overload has not been demonstrated. The policy specifically lists chelation for presumed mercury “toxicity” from dental amalgam fillings as unproven; high-quality evidence and randomized trials have not shown that dental amalgams cause harm except in rare allergy, and chelation for this indication is not supported.

Use of chelation therapy to prevent or treat cardiovascular disease or to improve symptoms of stable ischemic heart disease is excluded from routine use. Systematic reviews and randomized trials of EDTA chelation have not demonstrated consistent benefit for coronary artery disease or peripheral vascular disease, and major cardiology guideline statements recommend against using chelation to improve symptoms or reduce cardiovascular risk.

Multiple guideline and regulatory statements discourage using chelation for angina or for core symptoms of certain diseases. The Canadian Cardiovascular Society recommends that chelation should not be used to attempt to improve angina, NICE does not recommend chelation for core autism symptoms, and the FDA has warned that approved chelating agents are intended for specific toxicologic indications (e.g., lead poisoning, iron overload) and not for treating autism or claiming cures.

Chelation therapy is considered not medically necessary for chronic, progressive non-overload diseases — examples include cardiovascular disease, diabetes, cancer, Alzheimer’s disease, autism spectrum disorder, and Parkinson’s disease — because well-designed peer-reviewed studies do not support clinical benefit and safety concerns have been raised.
