Coverage Criteria
Initial therapy — Varenicline
Varenicline (generic Chantix) covered when ALL of the following are met:
Authorization issued for zero copay with deductible bypass for 3 months for new therapy
Initial therapy — Nicotrol NS / Inhaler
Nicotrol nasal spray or Nicotrol inhaler covered when ALL of the following are met:
Authorization issued for zero copay with deductible bypass for 3 months
UnitedHealthcare provides coverage for tobacco cessation pharmacotherapy with defined limits on therapy attempts. Kentucky requires coverage of two attempts, each defined as 180 days of therapy within a 12‑month period, without implementation of utilization management for those two attempts. After a member has received coverage for an attempt (180 days), any continued therapy beyond the two attempts must meet the policy’s coverage criteria for continuation or reauthorization.
For covered products in this policy, prior authorization is required. Approvals for new therapy are issued with a zero dollar copay and deductible bypass for 3 months; patients established on varenicline (evidence of a medication claim within the past 90 days) may receive an additional 3 months authorization with step therapy bypass. Claims may be approved or denied based on medication history, diagnosis codes, and claim logic when supporting documentation is not provided.

Step therapy applies unless member is currently on varenicline
Step therapy is required for initial approvals unless the member is currently on varenicline. Members currently on varenicline (established within past 90 days) bypass step therapy; otherwise, documentation of prior failure, contraindication, or intolerance to the listed OTC nicotine replacement therapies and to bupropion is required before approval.

Document and retain evidence supporting patient age (≥18), indication for tobacco cessation, prior failure/contraindication/intolerance to the specified OTC NRTs and to bupropion, and evidence of prior use when claiming an established-on-therapy exception (medication claim within the past 90 days).
