Therapeutic Duplication Edit — Approval Criteria

The requested medication subject to the therapeutic duplication edit will be approved when ONE of the following conditions is met:

Pharmacy or prescriber documentation must indicate discontinuation of the prior agent.
Special circumstances necessitate duplicate therapy (e.g., nationally recognized drug shortage)
Special circumstances must be documented.

Therapeutic Duplication Detection Rules

Therapeutic duplication edit applies when any of the following recent claim history patterns are detected:

Concurrent use with a different drug in the same therapeutic class per recent prescription claims history
Concurrent use with a different dosage of the same medication per recent prescription claims history
Concurrent use with a different drug in a different therapeutic class where both drugs share the same clinical indication but lack evidence-based support for concomitant use

Automated Approval / Re-approval

UnitedHealthcare may approve initial and reauthorization requests based solely on claim history, diagnosis codes and/or claim logic regardless of other coverage criteria.

Program Purpose and Scope

A concurrent DUR (cDUR) program screens all retail and mail prescription claims at the point of service against a member's prescription drug history to identify therapeutic duplication, dosing duplications, and potentially inappropriate concomitant use. The cDUR evaluates prescribing and utilization, including drug interactions, and uses claims edits and messages to the dispensing pharmacy to help ensure adjudicated prescriptions are clinically appropriate and to improve medication safety and cost effectiveness. When the system detects duplication patterns it applies a point-of-service claims edit (e.g., Reject 88) and may prompt communication or prior authorization requirements for the dispensing pharmacy.

This program applies to retail and mail dispensing workflows and affects dispensing pharmacies, prescribers, and UnitedHealthcare prior authorization processes for members. Providers and pharmacy staff should be prepared to respond to claims edits by documenting discontinuation of the prior agent or providing rationale for duplicate therapy (for example, a nationally recognized drug shortage). UnitedHealthcare may also use prior claim history, diagnosis codes (ICD-10), and claim logic to approve or reauthorize therapy automatically, and where prior authorization is applied the typical authorization duration is 2 weeks.
