Standards of medical practice and available evidence support patient self-administration (examples: FDA approved prescribing information, peer-reviewed medical literature, manufacturer-provided medical literature, evidence-based practice guidelines, compendia such as IBM Micromedex DRUGDEX or Clinical Pharmacology, and self-administration utilization statistics).

Clinician-administered dosage formulations (e.g., intravenous infusion) of drugs on the Self-Administered Medications List may be covered under the medical benefit when billed in clinician‑administered form.
