Coverage Criteria and Scope

Initial coverage criteria

Covered when ALL of the following are met:

Primary eligibility: The drug is approved by the U.S. Food and Drug Administration (FDA); and the requested drug is a covered benefit by the member's state Medicaid agency; and one of: (a) the requested drug is considered 'unproven' per UnitedHealthcare drug policy; or (b) the indication is not addressed by a UnitedHealthcare drug policy; or (c) no UnitedHealthcare drug policy exists for the requested drug.

Clinical and utilization requirements: The requested drug is intended to treat a chronic and seriously debilitating condition, or a Serious Rare Disease; the patient has not failed a previous course or trial of the requested drug; the patient is not currently enrolled in an eligible clinical trial; and there is documented history of failure, contraindication, or intolerance to standard/conventional therapies to treat or manage the disease or condition, where available.

Supporting evidence: Diagnosis is clinically supported by at least one of: (a) listed compendia — AHFS-DI Therapeutic Uses, Clinical Pharmacology Indications, or DRUGDEX (Micromedex) with Strength of Recommendation Class I, IIa, or IIb; or (b) clinical indications supported by InterQual Specialty Rx; or (c) two articles from major peer‑reviewed medical journals demonstrating the proposed off‑label use is generally safe and effective (unless validated, uncontested contradictory evidence appears in a major peer‑reviewed journal).

Examples of accepted journals and study designs are provided in policy (e.g., JAMA, NEJM, Lancet; randomized, double‑blind, placebo‑controlled trials among acceptable designs).

This policy does not address coverage of self‑administered medications covered under the pharmacy benefit; such products should be referenced to the pharmacy benefit coverage rules. It also excludes oncology medications covered under the medical benefit per the NCCN Drugs & Biologics Compendium (including radiopharmaceuticals and listed examples such as octreotide acetate, leuprolide acetate, leucovorin, and levoleucovorin); refer to the Medical Benefit Drug Policy titled Oncology Medication Clinical Coverage for those agents. Vaccines are likewise outside the scope of this policy.

This policy is intended to assist interpretation of UnitedHealthcare standard benefit plans but does not supersede federal, state, or contractual requirements. When differences exist, the applicable federal, state, or contractual terms govern. Before applying this policy to a coverage decision, review the specific federal, state, or contractual benefit terms. UnitedHealthcare may modify policies as needed, and third‑party tools (for example, InterQual Specialty Rx) may be used to assist in administering benefits; this policy is informational and not medical advice.

Use may be considered not medically necessary when any of the following apply: the requested drug is not FDA‑approved; the drug is not a covered benefit by the member's state Medicaid agency; the condition is not a chronic, seriously debilitating condition or a Serious Rare Disease; the required prior therapy documentation (history of failure, contraindication, or intolerance to standard/conventional therapies where available) is not provided; the patient is enrolled in an eligible clinical trial when the policy requires exclusion; or the diagnosis lacks required support from listed compendia, InterQual Specialty Rx, or the requisite peer‑reviewed literature.
