Coverage Criteria and Scope

Initial Medical Necessity Criteria

A specialty drug may be determined medically necessary for the requested off-label or unproven indication when ALL of the following are met:

Main coverage criteria: The drug is approved by the U.S. Food and Drug Administration (FDA);

Main coverage criteria: The requested drug is a covered benefit by the member's state Medicaid agency;

Main coverage criteria

ONE of: The requested drug is considered 'unproven' per UnitedHealthcare drug policy;
ONE of: The indication for the requested drug is not addressed by a UnitedHealthcare drug policy;
ONE of: A UnitedHealthcare drug policy does not exist for the requested drug;

Main coverage criteria: The requested drug is intended to treat a chronic and seriously debilitating condition, or a Serious Rare Disease;

Main coverage criteria: The patient has not previously failed a course or trial of the requested drug;

Main coverage criteria: The patient is not currently enrolled in an eligible clinical trial;

Main coverage criteria: There is documented history of failure, contraindication, or intolerance to standard, conventional therapies to treat or manage the disease or condition, where available;

This policy does not apply to drugs covered under the pharmacy benefit that are self‑administered; those medications are governed by the member’s pharmacy benefit plan. It also does not address oncology medications (including but not limited to octreotide acetate, leuprolide acetate, leucovorin, and levoleucovorin) or therapeutic radiopharmaceuticals when coverage is based upon the NCCN Drugs & Biologics Compendium®, and those therapies are managed under the Oncology Medication Clinical Coverage policy. Vaccines are likewise excluded from this policy.

Uses that do not meet all required criteria are not supported as medically necessary under this policy. Examples include when the drug is not FDA‑approved for any indication, the requested use is not a covered benefit by the member’s state Medicaid agency, the diagnosis lacks clinical support from an accepted compendium (AHFS‑DI, Clinical Pharmacology, or DRUGDEX Class I/IIa/IIb), InterQual Specialty Rx, or two peer‑reviewed journal articles, or when the request falls into an excluded category such as oncology therapies governed by the NCCN Compendium, vaccines, or pharmacy‑benefit self‑administered drugs.

Prior Authorization Required

Prior authorization is required when requesting medical-benefit specialty drugs for off‑label or unproven indications. The request must demonstrate that the drug is FDA‑approved, is a covered benefit by the member's state Medicaid agency, and that the patient meets the Indications of Coverage criteria (including chronic/seriously debilitating or serious rare disease, not currently in an eligible clinical trial, and documentation of prior failure/contraindication/intolerance to standard therapies where available).

Requests must include documentation of diagnosis support from accepted compendia or peer‑reviewed literature.
Requests for unproven or off‑label uses must show compendia or peer‑review support (AHFS‑DI, Clinical Pharmacology, DRUGDEX, InterQual Specialty Rx, or two peer‑reviewed articles).

Use of Third‑Party Criteria

UnitedHealthcare may use third‑party clinical criteria tools (for example, InterQual) to assist in administering benefits and processing prior authorization requests. These tools inform but do not replace the need to meet documented coverage criteria.

Dosing, route, frequency, or patient population that deviates significantly from the label are examples of off‑label use that may require review under the coverage criteria set forth in this policy.
