Bosulif will be approved based on the following criterion:

Diagnosis of chronic myelogenous / myeloid leukemia

Philadelphia chromosome-positive Acute Lymphoblastic Leukemia (Ph+ ALL) - Initial Authorization

Bosulif will be approved based on the following criterion:

Diagnosis of Philadelphia chromosome-positive acute lymphoblastic leukemia

Myeloid/Lymphoid Neoplasms with Eosinophilia and ABL1 rearrangement - Initial Authorization

Bosulif will be approved based on BOTH of the following criteria:

Diagnosis of myeloid/lymphoid neoplasms with eosinophilia

Presence of ABL1 rearrangement

General Pediatric Auto-approval

Covered when ALL of the following are met:

Patient is less than 19 years of age
