Medically Necessary Criteria for Minimally Invasive SIJ Fusion

Covered when ALL of the following are met:

Patient has chronic sacroiliac joint (SIJ) pain.

Patient has responded insufficiently to at least six months of non-surgical treatment consisting of non-steroidal anti-inflammatory drugs and/or opioids (if not contraindicated) and one or more of the following: rest, physical therapy, SIJ steroid injection.>= 6 months

Failure to respond means continued pain that interferes with activities of daily living and/or results in functional disability.

Other diagnoses that could be responsible for the patient's ongoing pain or disability have been ruled out (e.g., L5/S1 compression, hip osteoarthritis).

Providers must document evaluation that rule out additional or alternative diagnoses

Pain is confirmed with a diagnostic SIJ block.

Minimally invasive SIJ fusion is not indicated if pain is not confirmed with a diagnostic SIJ block.

Not Indicated / Contraindications for Minimally Invasive SIJ Fusion

Procedure is not indicated when ANY of the following are present:

Less than six months of back pain.< 6 months

Failure to pursue conservative treatment of the SIJ (unless contra-indicated).

Pain not confirmed with a diagnostic SIJ block.

Existence of other pathology that could explain the patient's pain.

Documentation Required

Document conservative treatment and response

Providers must document at least six months of non-surgical treatment and the patient's response. Documentation should show continued pain that interferes with activities of daily living or results in functional disability despite therapy.

Minimum 6 months of non-surgical treatment (e.g., NSAIDs and/or opioids if not contraindicated, rest, physical therapy, SIJ steroid injection)
