Minimally Invasive SIJ Fusion - Medical Necessity

Minimally invasive SIJ fusion (Titanium Triangular Implant System) is proven and medically necessary when ALL of the following are met:

Conservative therapy: Have undergone and failed a minimum 6 months of intensive nonoperative treatment that includes medication optimization (unless contraindicated), activity modification, and active therapeutic exercise targeted at the lumbar spine, pelvis, SIJ, and hip.6 months

NASS recommendations for confirming SIJ as pain generator and trial of corticosteroid injection

Fusion of the sacroiliac (SI) joint for back pain presumed to originate from the SIJ is considered not medically necessary when performed for indications other than those explicitly listed in this policy or when using devices other than the specified Titanium Triangular Implant System (iFuse). Documentation requirements described elsewhere (diagnostic confirmation, imaging, and conservative therapy) must be met for consideration of coverage; lacking those elements supports a determination of non‑coverage. Sacral lateral branch nerve blocks used solely as a diagnostic tool prior to radiofrequency ablation are also considered not medically necessary due to insufficient evidence of efficacy.

Minimally invasive SIJ fusion is not indicated for patients with less than 6 months of back pain or who have failed to pursue an adequate course of conservative treatment (unless there is a documented contraindication). Procedures should not be performed when the SIJ has not been confirmed as the pain generator by diagnostic testing, or when alternative pathology that could explain the patient’s pain has not been excluded.

Additional statements of non‑indication reinforce that minimally invasive SIJ fusion should not be performed if the patient has less than 6 months of symptoms, has not completed appropriate non‑surgical management, has no diagnostic confirmation of SIJ pain (e.g., diagnostic block), or when other diagnoses better explain the clinical presentation. These criteria align with ISASS recommendations for exclusionary conditions.

Sacral lateral branch nerve blocks performed as a diagnostic procedure prior to radiofrequency ablation are judged to be unproven and not medically necessary. The literature is sparse and insufficient to establish their diagnostic utility, and guidance documents and reviews cited in this policy do not support their routine use for diagnostic confirmation before RFA.

Use of alternative implant systems (for example, cylindrical threaded implants [CTIs] such as Rialto or other screw‑based devices) remains of uncertain benefit because comparative evidence is limited and of low quality. Published assessments and comparative cohort studies report variable outcomes and higher revision or complication rates in some CTI series versus the triangular implant (iFuse), and thus SIJ fusion with devices other than the Titanium Triangular Implant System is considered insufficiently supported by the evidence.

Sacroiliac joint dysfunction commonly presents with unilateral low back, buttock, or posterior pelvic pain that is typically nonradicular and maximal below L5. Diagnosis is based on history, targeted physical examination including a cluster of provocative tests (e.g., thigh thrust, compression, Gaenslen, Patrick’s/Fabere) that reproduce the patient’s typical pain, and imaging to exclude alternative causes. Imaging (plain radiographs and CT or MRI) should demonstrate SIJ degeneration or injury and exclude destructive lesions, infection, inflammatory arthropathy, or hip/spinal pathology. When the clinical exam and imaging are concordant but uncertainty remains, confirmation with image‑guided diagnostic intra‑articular SIJ injection demonstrating an acute ≥75% reduction in pain (for the expected duration of the anesthetic) supports that the SIJ is the pain generator and guides consideration of fusion after failure of conservative care.
