Conservative Treatment Requirements

Trial of appropriate pharmacotherapy prior to invasive procedures

Covered when ALL of the following are met:

Pharmacotherapy Trial: A trial of appropriate pharmacologic therapy (e.g., analgesic and/or antiepileptic agents for trigeminal neuralgia or other guideline-directed medical management for cancer-related pain) has been attempted and documented and the patient remains refractory to medical management, unless pharmacotherapy is contraindicated or not tolerated.

For trigeminal neuralgia, first-line medical management commonly involves antiepileptic and/or analgesic drugs; percutaneous neuroablation applies when TN is refractory to pharmacotherapy per InterQual criteria.
