Coverage Criteria
Products identified as Non-Formulary on the Essential Prescription Drug List are listed in this quick reference and may be excluded from coverage or subject to restrictions for Maine members. Examples in the guide include formulations of indomethacin and indocin® suspensions, certain lamotrigine (Lamictal®) and lamotrigine XR preparations, pregabalin (Lyrica) formulations, and gabapentin (Neurontin®) preparations. Providers should assume these non-formulary products are not routinely covered and consider formulary alternatives or prior authorization processes where applicable.
