Coverage Criteria — Medical Necessity and Exclusions

Diagnostic injections (initial and confirmatory)

Initial diagnostic and confirmatory facet joint/medial branch block injections are considered proven and medically necessary when ALL of the following are met:

Initial diagnostic block criteria: All of: pain exacerbated by facet loading maneuvers on physical exam (e.g., hyperextension, rotation); clinically significant improvement has not occurred (pain remains ≥3 on a 1–10 scale) after a minimum of 4 weeks of conservative care (including but not limited to pharmacotherapy, exercise, or physical therapy); clinical findings and imaging suggest no other cause of the pain (e.g., spinal stenosis with neurogenic claudication, disc herniation with radicular pain, infection, tumor, fracture, pain related to prior surgery); the spinal motion segment is not fused; and a radiofrequency joint denervation/ablation procedure is being considered.

Second (confirmatory) diagnostic block criteria: All of: administered at the same level and side as the initial block; the initial diagnostic injection produced a positive response demonstrated by relief for at least the expected minimum duration of the local anesthetic and functional improvement specific to the individual; and a radiofrequency joint denervation/ablation procedure is being considered.

Unproven and Not Medically Necessary

The following are unproven and not medically necessary due to insufficient evidence of efficacy:

Unproven / Not medically necessary indications: Any of: facet injections/medial branch blocks when a radiofrequency ablation is not being considered at the requested level(s); therapeutic injections for treating spinal pain after diagnostic injections have been completed; more than two facet joint injections/medial branch blocks at the same level and side (considered therapeutic); therapeutic facet joint injections and/or medial branch blocks for chronic spinal pain; a second diagnostic block if the initial injection did not confirm the joint as source of pain; in presence of untreated radiculopathy at the same level as the intended diagnostic injection (except radiculopathy caused by a facet joint synovial cyst); medial branch blocks with injection volume >0.5 mL; injections performed under ultrasound guidance; therapeutic injections at cervical, thoracic, or lumbar levels considered unproven and not medically necessary.
