Coverage Criteria for Epidural Steroid Injections (ESI)

Medical necessity for ESI

Epidural Steroid Injections (ESI) are proven and medically necessary when ALL of the following are met:

Primary criteria: The injection is intended for management of radicular pain as evidenced by history and physical exam.

Primary criteria: Radicular pain is unresponsive to conservative treatment for ≥ 4 weeks. Conservative treatment includes pharmacotherapy (for example, NSAIDs or acetaminophen), activity modification, or physical therapy/home exercise.>= 4 weeks

Document duration and components of conservative care.

Primary criteria: There is evidence of structural and/or functional nerve root involvement by imaging (e.g., MRI/CT) or electrodiagnostic studies.

Electrodiagnostic studies acceptable per policy.

Primary criteria: The injection is performed under fluoroscopic or CT guidance.

Ultrasound guidance is considered unproven and not medically necessary.

Limitations and subsequent injections

Coverage is subject to the following frequency limitations and reassessment criteria:

Session limits: Maximum of four (4) ESI sessions per region (cervical, thoracic, lumbosacral) per 12-month period starting from the date of the first approved injection; a session = one date of service.<=4 per region per 12 months

A region is cervical, thoracic, or lumbosacral.

Subsequent injection conditions: Subsequent ESIs beyond the limit may be provided only if radicular pain has returned or function has deteriorated AND one of the following is met:

Option A: Previous injection resulted in ≤ 50% pain relief or functional improvement for < 3 months as measured by validated tools, and there has been reassessment of the individual and injection site/technique.<=50% relief for <3 months

Lumbar radicular pain due to disc herniation: Strong evidence from systematic reviews and RCTs supports lumbar transforaminal ESIs for radicular pain due to disc herniation, with short-term treatment success often defined as ≥ 50% pain reduction (reported pooled success rates ~63% at 1 month and ~74% at 3 months in pooled analyses).>=50% pain reduction used as success metric in cited trials

Smith et al. systematic review reports 63% at 1 month and 74% at 3 months.

Thoracic and cervical indications: Some RCTs and observational studies support interlaminar and transforaminal ESIs for thoracic and cervical disc herniation or radicular pain, but the evidence base is smaller and less consistent than for lumbar disc herniation.

ASIPP and selected trials provide Level II evidence for some thoracic/cervical indications.

Spinal stenosis and nonradicular back pain: Guideline assessments (AHRQ, ACOEM, NASS, WFNS) find limited or inconsistent evidence for ESIs in spinal stenosis and nonradicular low back pain; steroid inclusion may not add benefit over local anesthetic alone for stenosis in some analyses and long-term benefit is generally limited.

Conditions that would contraindicate epidural steroid injections (ESI) include, but are not limited to: spinal neoplasm, rapidly progressing neurological deficit, and epidural abscess. These conditions should prompt clinicians to avoid ESI and pursue appropriate urgent evaluation and management.

The use of ultrasound guidance for ESIs is considered unproven and not medically necessary because available peer‑reviewed evidence does not demonstrate superior or clinically meaningful benefit compared with fluoroscopic or CT guidance. Systematic reviews and RCT meta‑analyses report no consistent advantage in pain or disability outcomes and note substantial heterogeneity and risk of bias in the literature; therefore fluoroscopy or CT guidance remain the supported imaging modalities for covered ESIs.

ESIs provided for spine indications other than the management of documented radicular pain as described in the coverage criteria are considered not medically necessary. The policy restricts coverage to interventions intended to treat radicular symptoms with supporting clinical and imaging or electrodiagnostic evidence.

Professional guidance (ACOEM, AAN) and evidence reviews do not recommend routine use of ESIs in the absence of significant radicular symptoms. ACOEM recommends ESIs for select acute or subacute radicular pain after at least ~3 weeks of conservative care but lists ESIs as Not Recommended for acute, subacute, or chronic low back pain without significant radiculopathy. AAN similarly notes only limited, short‑term benefit for radicular lumbosacral pain and does not support routine use for non‑radicular low back pain.

As reiterated elsewhere in this policy, ultrasound‑guided ESIs are classified as unproven and not medically necessary due to insufficient evidence of efficacy, and ESIs delivered for non‑radicular indications (i.e., back pain that does not radiate along a dermatome or lacks evidence of nerve‑root compression) are considered not medically necessary.

Evidence for ESIs in nonradicular low back pain and for spinal stenosis is limited and inconsistent; systematic reviews and major society assessments report small, short‑term benefits at best and no sustained long‑term effectiveness. AHRQ and other assessments found that benefits for radiculopathy are immediate but not long‑term, and multiple guideline bodies (AAN, NASS, ACOEM) note insufficient evidence to support routine use of ESIs for nonradicular low back pain or spinal stenosis.
