Coverage Criteria for Balloon Sinus Ostial Dilation and FESS

CRS — Balloon sinus ostial dilation and/or FESS

Covered when ALL of the following are met

CRS duration: CRS lasting longer than 12 weeks>12 weeks

Prior medical management: Persistence of symptoms despite recent medical management with full courses of: intranasal corticosteroids (and/or oral corticosteroids when appropriate); antibiotic therapy if bacterial infection is suspected; and nasal lavage/irrigation if appropriatedocumented trial

Recent CT confirmation: Documentation of CT for each sinus to be treated obtained after completion of medical management including identification of sinus and extent of disease (percent opacification or Modified Lund‑Mackay scoring); CT findings must include one or more: bony remodeling, bony thickening, opacified sinus, ostial obstruction and mucosal thickeningCT within 12 months

Symptom concordance: Sinonasal symptoms such as pain, pressure, or drainage are present on the same side as CT scan findings of rhinosinusitis

Balloon dilation anatomic limit: For balloon sinus ostial dilation only, dilation is limited to the frontal, maxillary, or sphenoid sinuses

FESS — Additional indications

FESS is also covered when ANY of the following CT-confirmed conditions are present

FESS additional criteria: Complications of sinusitis (e.g., abscess); symptomatic concha bullosa; symptomatic mucocele; polyposis with obstructive symptoms (for CRSwNP refer to CRS criteria above); sinonasal tumor

For CRS with polyps, refer to CRS criteria above

RARS — Coverage criteria

Covered when ALL of the following are met

Episode frequency: Four or more episodes per year with distinct symptom‑free intervals between episodes>=4 episodes/year

Symptom concordance: Sinonasal symptoms such as pain, pressure, or drainage are present on the same side as CT scan findings of rhinosinusitis

Recent CT evidence: CT demonstrating ostial obstruction and mucosal thickening for BSD; for FESS, ostial obstruction and mucosal thickening for maxillary/frontal/sphenoid and mucosal thickening for ethmoidCT within 12 months

Medical necessity criteria for BSD

Covered when ALL of the following are met

Clinical diagnosis and prior therapy: Clinical diagnosis of CRS or RARS based on symptoms and failure of appropriate medical therapy (e.g., intranasal corticosteroids, saline irrigation; antibiotics as appropriate)

Society guidance indicates medical therapy should precede procedural intervention.

Objective documentation: Objective confirmation by nasal endoscopy documenting sinonasal abnormality OR CT demonstrating mucosal thickening or ostial occlusion/opacification

AAO‑HNS, ARS, and imaging guidance (ACR) state CT or endoscopy is required prior to BSD.

Appropriate patient selection: Selected patients with limited CRS without severe nasal polyposis or RARS where BSD is expected to address ostial obstruction; BSD may be used alone or adjunctively with FESS at surgeon discretion

NICE supports XprESS for uncomplicated CRS without severe polyposis; AAO‑HNS and ARS provide similar selection guidance.

Indications for ESS / Coverage criteria

Covered when ALL of the following are met:

Primary criteria: 1) Persistent sinonasal symptoms consistent with CRS/CRSwNP despite appropriate/maximal medical therapy; 2) Objective evidence of disease on nasal endoscopy and/or CT imaging; 3) Documentation of symptom burden (e.g., SNOT‑22) and clinical indication for surgery (e.g., obstruction, recurrent/persistent infection, complications, mucocele, inverted papilloma); 4) CT imaging obtained when ESS is considered or planned.

Randomized controlled evidence

Relative benefit demonstrated in RCT evidence:

RCT evidence: In an RCT of adults with CRSwNP (Lourijsen et al. 2022), ESS plus medical therapy produced greater improvement in SNOT‑22 at 12 months versus medical therapy alone, though the between‑group difference did not reach the study’s minimal clinically important difference; this supports benefit of ESS when criteria are met.

Comparative outcomes by extent of surgery

Evidence on extent and outcomes:

Extent of ESS: Systematic reviews and meta‑analyses report that greater extent of ESS (e.g., full‑house, extended, radical) is associated with larger improvements in patient‑reported outcome measures (SNOT‑22) and reduced revision rates compared to limited ESS, with similar perioperative complication rates across extents; heterogeneity and inconsistent definitions limit firm recommendations.

Tumor and neoplasm indications

Specific disease considerations:

Sinonasal malignancy / inverted papilloma: Endoscopic approaches offer comparable oncologic outcomes for select low‑stage sinonasal malignancies and are associated with lower recurrence for inverted papilloma versus external approaches; approach selection should be based on tumor stage and anatomy.

National Cancer Database and pooled case‑series/meta‑analyses support endoscopic alternatives in appropriate cases

Balloon sinus ostial dilation (BSD) is unproven and not medically necessary for treatment of sinonasal polyps or tumors, and for cases of chronic rhinosinusitis (CRS) or recurrent acute rhinosinusitis (RARS) that do not meet the policy’s stated clinical criteria. Functional endoscopic sinus surgery (FESS) is likewise considered unproven and not medically necessary when CRS or RARS cases do not satisfy the coverage criteria in this policy. Additionally, self‑expanding absorptive sinus ostial dilation devices are considered unproven and not medically necessary for evaluation or treatment of sinusitis and other conditions because current evidence is insufficient to demonstrate efficacy.

Balloon dilation is not appropriate for patients who do not have both sinonasal symptoms and positive CT findings; CT scanning of the sinuses is required before balloon dilation. BSD is not appropriate for management of headache (including barometric pressure–related sinus headache) or sleep apnea in patients who do not otherwise meet CRS or RARS criteria. Randomized trial evidence has not shown benefit of BSD over placebo for patients selected for headache without radiographic mucosal disease.

Self‑expanding absorptive sinus ostial dilation devices have limited evidence consisting primarily of small pilot studies without controlled comparison groups. Available reports demonstrate feasibility and short‑term ostial patency in small samples, but do not provide sufficient comparative effectiveness or safety data to support routine clinical use. Therefore the evidence is currently insufficient to support routine use of these self‑expanding absorptive devices for sinus ostial dilation.

FDA clearance or device listing is informational only and is not by itself a basis for coverage. The FDA classifies many dilation instruments under product code LRC and some devices have received 510(k) clearance (for example, the SinuSys Vent‑OS system in 2013). FESS is a surgical procedure and therefore the procedure itself is not subject to FDA premarket regulation, although any devices, drugs, biologics, or tests used during the procedure may be subject to FDA regulation.

Coverage decisions are governed by applicable federal, state, and contractual benefit plan requirements; this policy is informational and does not guarantee coverage. Medical records documentation may be required to assess whether a member meets the clinical criteria for coverage, but documentation alone does not ensure that the requested service will be covered. Refer to the guideline titled 'Medical Records Documentation Used for Reviews' and verify applicable plan or legal requirements before applying this policy.

When members do not meet the policy’s defined clinical criteria, FESS is considered unproven and not medically necessary. The same determination applies to use of self‑expanding absorptive sinus ostial dilation devices, which lack adequate comparative evidence to support their use; such devices are therefore treated as unproven/insufficient for coverage.

Use of BSD solely to treat barometric pressure–related sinus headache or rhinogenic headache is not supported unless there is objective evidence of inflammatory sinonasal disease on CT or endoscopy. A blinded randomized trial found no difference between active sinus ostial dilation and a nasal placebo procedure in patients selected for sinus pressure headache without CT evidence of mucosal thickening; therefore BSD should not be used alone for headache indications in the absence of documented inflammatory disease.

Self‑expanding absorptive sinus ostial dilation devices are judged to have insufficient evidence to support coverage. Small pilot series show feasible placement and short‑term ostial dilation, but the studies lack control groups and provide limited safety and outcome data; current evidence does not support routine coverage for these devices.

Prior authorization is required for applicable procedures (including balloon sinus dilation and endoscopic sinus surgery) when billing referenced CPT codes. Authorization requests must document that the patient failed appropriate medical therapy and that objective evidence of sinonasal disease is present (CT and/or nasal endoscopy) to support medical necessity.

Authorization for balloon sinus dilation (BSD) requires clinical and objective evidence that BSD is appropriate for the member. BSD is a therapeutic option for selected patients who have failed appropriate medical therapy and have supporting endoscopic or CT findings.

Background and Clinical Context

Chronic rhinosinusitis (CRS) is an inflammatory condition of the paranasal sinuses that persists for more than 12 weeks and is characterized by two or more symptoms such as mucopurulent drainage, nasal obstruction, facial pain/pressure/fullness, or decreased sense of smell. FESS is a minimally invasive surgical approach to visualize and open diseased sinuses and is an established option for medically refractory CRS; balloon sinus ostial dilation is an alternative or adjunctive technique that dilates sinus ostia and may be used alone or with FESS in appropriately selected patients.
