State-Specific Medical Necessity for Balloon Ostial Dilation and FESS

Covered when ALL of the following are met:

CRS location and duration: Uncomplicated chronic rhinosinusitis limited to the paranasal sinuses without involvement of adjacent neurological, soft tissue, or bony structures that has persisted for at least 12 weeks>= 12 weeks

Must be limited to paranasal sinuses

Symptom requirement: At least two of the following sinonasal symptoms: facial pain/pressure; hyposmia/anosmia; nasal obstruction; mucopurulent nasal discharge>=2 symptoms

Failure of maximal medical therapy: Sinonasal symptoms are persistent after maximal medical therapy has been attempted, defined by saline nasal irrigation for at least six weeks; nasal corticosteroids for at least six weeks; approved biologics, if applicable, for at least six weeks; a complete course of antibiotic therapy when an acute bacterial infection is suspected; and treatment of concomitant allergic rhinitis if presentvarious >=6 weeks

Stance: Self-expanding absorptive sinus ostial dilation is considered unproven and not medically necessary for evaluating or treating sinusitis and all other conditions due to insufficient evidence of efficacy

Includes devices such as the SinuSys Vent-OS; evidence limited by small studies lacking comparison groups

Balloon ostial dilation and functional endoscopic sinus surgery are not covered and not considered medically necessary in specified situations. These exclusions include: absence of objective evidence of sinonasal disease on nasal endoscopy or computed tomography despite sinonasal symptoms; use for obstructive sleep apnea and/or snoring when the policy criteria are not met; treatment of headaches when the policy criteria are not met; and balloon ostial dilation when sinonasal polyps are present. Providers should ensure objective findings and that eligibility criteria are met before submitting claims or prior authorization requests to avoid denial for these reasons.

Self-expanding absorptive sinus ostial dilation is considered not medically necessary because the available evidence is insufficient to demonstrate efficacy for evaluation or treatment of sinusitis or other conditions. Published studies are limited by small sample sizes and lack of comparison groups, and do not provide adequate data to support routine clinical use.

One example cited in the literature is a small pilot study of a maxillary sinus ostium self-dilation device that enrolled a limited number of patients and lacked a control group; although the device was reported to dilate ostia and had few adverse events, these methodological limitations leave its effectiveness unproven. Consequently, self-expanding absorptive sinus ostial dilation (including devices such as those evaluated in small pilots) remains not medically necessary under this policy.
