Coverage and Medical Necessity Criteria

Nasal valve procedures / repair criteria

Nasal valve procedures/repair of nasal vestibular stenosis or alar collapse are considered reconstructive and medically necessary when ALL of the following criteria are met:

Primary symptom: Prolonged, persistent obstructed nasal breathing due to internal and/or External Nasal Valve compromise

Exclusion of other causes: Other causes of nasal obstruction (e.g., rhinosinusitis, allergic rhinitis, vasomotor rhinitis, nasal polyposis, adenoid hypertrophy, nasopharyngeal masses) have been adequately treated with maximal therapy and nasal obstruction persists

Septal/turbinate status: Nasal septal deviation and turbinate hypertrophy are either not present OR have been previously surgically treated OR are scheduled to be surgically treated at the same time as the nasal valve procedure

Objective/clinical documentation: Documented visible collapse of alar (lower lateral) cartilage and/or lateral nasal wall with deep inspiration; documented subjective and audible improvement in airflow during modified Cottle maneuver; and photos clearly documenting dynamic collapse or anatomic deformity consistent with clinical exam

Surgical plan: Surgeon description that states whether compromise is static or dynamic, whether internal and/or External Nasal Valve is involved, and a clear surgical plan including need for cartilage graft when applicable

Rhinophyma excision criteria

Rhinophyma excision is considered reconstructive and medically necessary when ALL of the following are present:

Examples include absorbable polylactic acid nasal cartilage support implants (e.g., Latera), various nasal septal swell body (NSB) reduction techniques, and device‑based rhinitis interventions such as PNN/SPG ablation (RhinAer, ClariFix). Because most studies are small, nonrandomized, or lack durable follow‑up and head‑to‑head comparators, these procedures are considered unproven for routine coverage in the absence of robust supporting documentation.

Key limitations in the evidence base that inform coverage decisions include short follow‑up durations, frequent absence of active comparator arms, small sample sizes, and a preponderance of observational or industry‑sponsored studies with higher risk of bias.
