Coverage and Medical Necessity Criteria

Nasal valve procedures — medical necessity

Nasal valve procedures/repair of nasal vestibular stenosis or alar collapse are considered reconstructive and medically necessary when ALL of the following are present:

Nasal valve medical necessity criteria: Other causes of nasal obstruction (eg, rhinosinusitis, allergic rhinitis, vasomotor rhinitis, nasal polyposis, adenoid hypertrophy, and/or nasopharyngeal masses) have been adequately treated with maximal therapy and nasal obstruction persists.

Duration/Type of obstruction: Prolonged, persistent obstructed nasal breathing due to internal and/or external nasal valve compromise.

Septal/turbinate status: Nasal septal deviation and turbinate hypertrophy either have been previously surgically treated; or are not present; or are scheduled to be surgically treated at the same time as the nasal valve procedure.

Objective exam: Documented evidence of visible collapse of the alar (external nasal valve) and/or lateral nasal wall (internal nasal valve) with deep inspiration.

Functional maneuver: Documented evidence of subjective and audible improvement in nasal airflow during modified Cottle maneuver.

Photographic documentation: Photos clearly document dynamic collapse of the internal and/or external nasal valve or anatomic deformities narrowing the internal and/or external nasal valve as a main cause of mechanical nasal airway obstruction and are consistent with clinical exam.

Surgical plan details: Surgeon has described whether the nasal valve compromise involves internal, external, or both; whether it is static or dynamic; and provided a clear surgical plan including the need for cartilage graft.

Rhinophyma excision — medical necessity

Rhinophyma excision is considered reconstructive and medically necessary when ALL of the following are present:

Rhinophyma indication: Prolonged, persistent obstructed nasal breathing due to rhinophyma; or chronic infection or bleeding unresponsive to medical management due to rhinophyma.

Photographic and procedural intent: Photos clearly document rhinophyma as the primary cause of an anatomic mechanical nasal airway obstruction or chronic infection and are consistent with the clinical exam; and the proposed procedure is designed to correct the anatomic obstruction or address the chronic infection.

Nasal valve procedures (including repair of nasal vestibular stenosis or alar collapse) are considered reconstructive and medically necessary only when all specified clinical criteria are met. These criteria require documentation that other causes of nasal obstruction have been adequately treated and that the patient has prolonged, persistent obstructed nasal breathing due to internal and/or external nasal valve compromise. The exam must show visible dynamic collapse of the alar (external nasal valve) and/or lateral nasal wall (internal nasal valve) with deep inspiration, and the modified Cottle maneuver must demonstrate subjective and/or audible improvement in airflow. Photographic documentation consistent with the clinical exam and a clearly described surgical plan (including whether the compromise is internal, external, or both; static or dynamic; and whether a cartilage graft is needed) are required. Procedures not meeting these criteria are not considered reconstructive or medically necessary under this policy.

For rhinophyma excision to be considered reconstructive and medically necessary, the procedure must be performed for prolonged, persistent obstructed nasal breathing due to rhinophyma, or for chronic infection or bleeding from rhinophyma that is unresponsive to medical management. Photographs must clearly document rhinophyma as the primary cause of the mechanical nasal airway obstruction or chronic infection and be consistent with the clinical exam, and the proposed procedure must be intended to correct the anatomic obstruction or address the chronic infection. For broader rhinoplasty/septoplasty medical necessity criteria, the policy refers to the North Carolina Medicaid Clinical Coverage Policy on Reconstructive Surgery: Rhinoplasty and/or Septoplasty.

The AAO‑HNS definition of rhinoplasty recognizes that when adjunctive procedures on the septum, nasal valve, or turbinates are performed without altering nasal shape or appearance, they do not meet the definition of rhinoplasty. Accordingly, procedures performed without any impact on nasal shape or appearance — i.e., those that do not meet the guideline definition of rhinoplasty — are excluded from rhinoplasty‑specific guideline considerations and are not evaluated under the rhinoplasty criteria in this policy.

Evidence for septal swell body (NST/NSB) interventions is mixed. Some studies using cautery or cryosurgery did not show symptomatic or rhinometric benefit (Haight et al.), while other prospective studies of RFA or coblation reported improved NOSE scores. These inconsistent findings indicate that not all energy‑based modalities demonstrate consistent benefit for NST/NSB reduction.

Overall, the evidence base for many nasal airway procedures is limited by short follow‑up times, small and heterogeneous study designs, and few head‑to‑head comparisons. For example, RCTs of PNN neurolysis report short‑term responder advantages versus sham at three months but lack long‑term comparative data; reviews of VivAer (TCRF) and other radiofrequency approaches note consistent short‑term improvements but emphasize very‑low to low quality evidence and insufficient longer‑term comparative effectiveness data versus established surgical treatments.

This policy is provided for informational purposes to help interpret UnitedHealthcare standard benefit plans. Federal, state, or contractual benefit plan requirements take precedence over this policy; in the event of conflict, those requirements govern. Providers should verify applicable plan terms before using this policy.

The document lists procedures considered unproven and not medically necessary due to insufficient evidence of safety and/or efficacy. These include nasal septal swell body (NSB) reduction for nasal obstruction, absorbable polylactic acid nasal cartilage support implants (for example, the Latera implant), and posterior nasal nerve or sphenopalatine ganglion ablation by any method (such as radiofrequency or cryoablation, e.g., RhinAer, ClariFix) for treatment of chronic rhinitis.

Absorbable nasal implants such as Latera have shown promising results in nonrandomized studies, but systematic and evolving evidence reviews (Hayes) conclude the overall quality is low. Key limitations include small cohorts, heterogeneous adjunctive procedures, limited control‑group data, and inadequate long‑term follow‑up. Because of these limitations, use of absorbable nasal implants for indications lacking long‑term comparative benefit versus established surgical techniques remains unproven.

Hayes evolving evidence reviews for ClariFix and RhinAer found minimal or unclear support for these PNN/SPG ablation technologies. The reviews identified limited-quality studies (including some RCT data) but concluded current evidence does not permit definitive conclusions that these procedures produce better, worse, or equivalent outcomes compared with other treatments at this time.

Evidence for the VivAer temperature‑controlled RF stylus includes one RCT and several pre‑post studies showing clinically significant improvements in nasal breathing at three months and up to 24 months in some reports. However, authoritative assessments (ECRI, Hayes) characterize the overall evidence as very‑low to low quality and emphasize a lack of robust head‑to‑head comparative trials versus established surgical options; therefore, comparative effectiveness remains uncertain.
