Covered when ALL of the following are met:

Nasal valve procedures - medical necessity criteria: Prolonged, Persistent Obstructed nasal breathing due to internal and/or External Nasal Valve compromise; AND Other causes of nasal obstruction (e.g., rhinosinusitis, allergic rhinitis, vasomotor rhinitis, nasal polyposis, adenoid hypertrophy, nasopharyngeal masses) have been adequately treated with maximal therapy and obstruction persists; AND Nasal septal deviation and turbinate hypertrophy are not present, or have been previously surgically treated, or are scheduled to be treated at the same time as the nasal valve procedure; AND Documented visible collapse of alar (lower lateral) cartilage and/or lateral nasal wall with deep inspiration; AND Documented subjective and audible improvement in nasal airflow during a modified Cottle maneuver; AND Photographs clearly document dynamic collapse or anatomic deformities narrowing the internal and/or External Nasal Valve as the main cause and are consistent with the clinical exam; AND The surgeon has clearly described whether the valve compromise is static or dynamic, which valve(s) are involved (internal, External, or both), and a plainly stated surgical plan including the need for cartilage graft

Guidance from specialty organizations concerning indications and timing for nasal surgery:

Specialty guidance: Surgical repair of the nasal valve can improve nasal obstruction symptoms for appropriately selected patients; surgical approaches may include cartilage grafting, open repair, suture suspension techniques, implants, or radiofrequency methods and should address other causes of obstruction to optimize outcomes.

Evidence summaries on effectiveness:

Functional rhinoplasty: Systematic reviews and meta-analyses report significant improvements in NOSE and VAS scores up to 12 months (and in some studies beyond) after functional rhinoplasty addressing nasal valve problems, but heterogeneity and study limitations (observational designs, variable follow-up) exist.

Evidence summary for posterior nasal nerve (PNN) / sphenopalatine ganglion (SPG) ablation

PNN/SPG evidence: Randomized, sham-controlled trials and multicenter studies report clinically meaningful short-term reductions in symptom scores for PNN/SPG ablation (both RF neurolysis and cryotherapy) with some studies reporting durable improvements to two years, but limitations include heterogenous rhinitis types, variable medication management, and lack of long-term controlled comparisons; overall evidence insufficient to fully establish long-term superiority over established therapies.

inv-13: Coverage criteria for PNN/SPG ablation

Procedural treatments of the posterior nasal nerve (cryotherapy, radiofrequency) are considered when ALL of the following are met:

PNN/SPG ablation candidate criteria: Chronic rhinitis symptoms present for ≥4–6 months; AND Inadequate response to appropriate medical therapy (intranasal steroids and/or antihistamines/anticholinergics) for at least 4 weeks; AND Baseline validated symptom score meeting trial thresholds (commonly rTNSS ≥4–6 with rhinorrhea subscore ≥2); AND Patient has been informed of expected benefits and risks and provided informed consent; AND Procedure is performed using a validated device by a qualified provider.rTNSS commonly >=4–6 in trials

inv-14: Coverage criteria for Nasal Septal Swell Body (NSB) reduction

Septal body reduction (SBVR) considerations:

NSB reduction candidate criteria: Nasal obstruction associated with septal body and inferior turbinate hypertrophy refractory to medical therapy; AND Clinical assessment supports septal body hypertrophy contributing to obstruction; AND SBVR may be performed concomitantly with inferior turbinoplasty as part of the surgical plan.

Randomized short-term study showed greater improvement with combined SBVR+ITR vs ITR alone but lacked long-term data.

inv-15: Coverage considerations for TCRF nasal valve treatment

TCRF treatment of nasal valves:

TCRF nasal valve treatment considerations: Candidate has nasal valve dysfunction causing airway obstruction; AND TCRF may be considered as a less invasive alternative to functional rhinoplasty when appropriate; AND Patient is informed that long-term, multi-center randomized trial data are limited and that evidence is heterogeneous across studies.

Systematic reviews/meta-analyses indicate comparable short-term outcomes to rhinoplasty but highlight heterogeneity and limited high-quality RCT evidence.

inv-16: TCRF / Nasal valve treatment - conditional coverage logic

TCRF (nasal valve) coverage considerations: Patient has symptomatic nasal airway obstruction due primarily to internal nasal valve collapse or nasal valve dysfunction; AND Baseline NOSE score consistent with trial eligibility (eg, NOSE >=60 used in many cohorts); AND Documented trial of conservative management (medical therapy and/or nasal dilators) with persistent symptoms; AND Clinician documents expected benefit using trial responder definitions (eg, >=20% NOSE improvement or >=1 severity class); AND Patient counseled regarding trial evidence (short-term RCT benefit and some sustained cohorts to 24 months) and limitations (single-arm designs, small samples, industry sponsorship).NOSE >=60 commonly used in cohorts

PNN ablation / cryotherapy: Randomized, sham-controlled RCTs and society position statements support posterior nasal nerve neurolysis (radiofrequency or cryotherapy) for medically refractory chronic rhinitis; documentation should show failure of appropriate medical therapy and symptom burden consistent with trial populations.failure of medical therapy

Procedures are not considered reconstructive and medically necessary when the indication does not meet all of the specific criteria listed for nasal valve procedures, repair of nasal vestibular stenosis, or alar collapse. Covered reconstructive indications require documentation of prolonged, persistent obstructed nasal breathing due to internal and/or external nasal valve compromise, prior adequate treatment of other causes of obstruction, assessment and treatment status of septal deviation and turbinate hypertrophy, visible dynamic or static collapse on exam, improvement with a modified Cottle maneuver, photographic documentation consistent with the clinical exam, and a clear surgical plan including whether a cartilage graft is needed.

Adjunctive procedures that do not alter nasal shape or appearance are excluded from the AAO‑HNS guideline definition of rhinoplasty. When septal, turbinate, or other adjunctive interventions are performed without impact on nasal form, they do not meet the definition of rhinoplasty and therefore are not considered rhinoplasty under the guideline.

Procedures may be considered investigational when the published evidence is of low quality, lacks controlled comparative trials, or when outcomes are confounded by concurrent adjunctive procedures. Systematic reviews and evidence assessments have noted many studies are single‑arm or technique‑driven, limiting conclusions about patient benefit versus established therapies.

Hayes and ECRI reviews highlight limited long‑term data and the need for additional randomized controlled trials. Reviews of absorbable implants and radiofrequency devices conclude that study quality is generally poor or limited and that longer‑term, high‑quality RCTs are required before strong conclusions about sustained benefit can be drawn.

Procedures performed solely for cosmetic improvement of nasal form, or when nasal obstruction is not substantiated by symptoms or validated outcome measures, are outside the therapeutic indications described in this policy and are not considered reconstructive or medically necessary.

Hayes reviews concluded there is minimal or unclear support for certain cryoablation devices (eg, ClariFix) and that the RhinAer evidence base is insufficient to determine whether outcomes are better, worse, or similar to alternative treatments. These evidence limitations may affect coverage decisions and authorization requirements.

Use of temperature‑controlled radiofrequency devices (TCRF/VivAer) for nasal valve remodeling should be considered cautiously when comparative effectiveness versus functional rhinoplasty or other surgical interventions has not been demonstrated by high‑quality randomized trials. Meta‑analyses and reviews note promising short‑term effects but emphasize heterogeneity and lack of robust long‑term RCT data.
