AAO-HNS (2015, reviewed 2021) considers FDA-approved biomaterials for rhinologic application not investigational; use to be determined by treating physician based on evidence, surgeon experience, clinical situation, and patient preference.

American Rhinologic Society (ARS, 2022) supports use of bioabsorbable nasal implants to treat nasal obstruction due to nasal valve collapse and considers it an effective option based on cited studies.

Reported study findings (select examples): Pritikin et al. (2023; 2024 follow-up): prospective, open-label study of TCRF (VivAer) for SSB hypertrophy (n=70) showed significant NOSE score improvement at 3, 6, and 12 months; responder rate 95.7% at 3 months; limitations: open-label, single-arm, short-term follow-up.

Yu et al. (2015): prospective randomized trial (n=51) comparing microdebrider-assisted SBVR + ITR versus ITR alone showed greater improvement in nasal obstruction and volume at 3 months; follow-up limited to 3 months.

Systematic reviews (Meng 2021; Moss et al. 2019) found promising but limited evidence; small sample sizes, short follow-up, heterogenous designs; NST RFA, coblation, and submucosal reduction reported as safe and effective in included small studies.

Kim (2016), Catalano, Haight studies: small cohorts or non-randomized designs report symptom improvements; complications rare but include one septal perforation in Catalano series.

Posterior Nasal Nerve (PNN) / Sphenopalatine Ganglion (SPG) ablation - evidence summary

Multiple RCTs, prospective single-arm studies, systematic reviews, and evolving evidence reviews summarized; collective evidence shows short-term symptom improvement with limitations noted:

Randomized controlled trial findings: Stolovitzky et al. (2021): multicenter, patient-blinded RCT (RhinAer TCRF vs sham) with rTNSS ≥6 entry; active treatment superior to sham at 3 months (responder rate 67.5% vs 41.0%) and greater decrease in rTNSS; limitations: short follow-up (3 months), pragmatic medication use, lack of long-term data.

Prospective and observational study findings: Lee et al. (2022): multicenter single-arm TCRF neurolysis (RhinAer) in 129 adults showed mean rTNSS improvement 53.8% at 3 months and 62.8% at 6 months; no major adverse events; lack of control arm. Ow et al. (2021): prospective single-arm cryoablation study (n=100) with improvements in rTNSS and RQLQ sustained to 24 months in subsets; single-arm design, attrition noted. Ehmer et al. (2022): single-arm TCRF PNN neurolysis with 52-week follow-up (n=50, 47 completed) showed mean rTNSS improvement from 8.5 to 3.6 at 52 weeks; limitations: no control/blinding. Takashima et al. (2023/2024): RCT with cross-over and extended follow-up showed sustained improvements to 12–24 months but limitations include loss of sustained control arm and medication management issues.

Systematic review conclusions: Systematic reviews/meta-analyses (Balai 2023; Young 2023; Desai 2023; ECRI 2020 updated 2023) generally report symptom improvement and safety (mostly transient, non-serious AEs) but emphasize small number of RCTs, short follow-up, heterogeneity, risk of bias, and need for further high-quality RCTs comparing these interventions to other treatments.

Policy conclusion language in this section: 'quality of the body of evidence for PNN and SPG ablation is insufficient to demonstrate benefit on long-term patient outcomes compared to established therapies' and 'additional prospective RCTs with larger numbers and medium to long-term follow-up are recommended.'
