Clinical and Administrative Background

Lumbar spinal stenosis (LSS) is a degenerative narrowing of the lumbar spinal canal that can compress neural elements and produce back and leg symptoms. Degenerative changes usually begin with disc breakdown and collapse of disc height, with transferred load to the facet joints causing cartilage erosion, osteophyte formation and ligamentous thickening; while trauma, tumor, infection, or congenital factors can contribute, degeneration is the predominant cause. Initial management is conservative—rest, NSAIDs, muscle relaxants, corset use, physical therapy, and lumbar epidural steroid injections—with surgery considered when symptoms are moderate to severe or refractory to these measures and quality of life is substantially impaired. Surgical options for moderate to severe or refractory LSS primarily include neural decompression procedures with or without spinal fusion (arthrodesis). Decompression removes offending bone, ligament, and disc material (for example via laminectomy, laminotomy/hemilaminectomy, facetectomy, foraminotomy, or endoscopic techniques) to relieve pressure on the neural elements. Fusion joins two or more vertebrae to treat instability or severe degeneration, typically by removing the disc and placing bone grafts or spacers into the interspace so bony bridging can occur. Fusion procedures can be performed using different approaches (anterior, posterior, lateral/extracavitary) and techniques (posterior interbody, posterior/posterolateral, anterior interbody, combined approaches), and are often supplemented with instrumentation (plates, rods, pedicle screws, cages) to stabilize the construct while fusion matures.

The most common lumbar fusion approach is the posterior approach, where exposure from the back allows decompression followed by placement of bone graft and supplemental fixation with metal screws and rods to prevent motion during healing. Alternatives include anterior and lateral techniques and combined procedures; spacers used for interbody fusion may be made of bone, titanium, carbon fiber, polymers or bioresorbable materials and are frequently supported by plates, screws, rods, and/or cages. Fusion is most often indicated to address instability, severe degenerative disc disease with neurologic compromise, fractures, infection, or tumor-related vertebral destruction, recognizing that fusion and decompression carry increased operative risk, particularly in older patients.
