InterQual-driven coverage: Clinical coverage determinations for decompression +/- fusion and fusion procedures (cervical, thoracic, lumbar) and scoliosis/kyphosis surgery follow the referenced InterQual® CP procedure criteria (Decompression +/- Fusion, Fusion, and Scoliosis/Kyphosis categories).

Procedures considered unproven / not medically necessary

Not medically necessary when listed.

Unproven procedures: Isolated facet fusion (with or without instrumentation), dynamic stabilization systems for degenerative spondylolisthesis, and total facet joint arthroplasty are considered unproven and not medically necessary due to insufficient evidence of efficacy.

Declared unproven because available evidence is inadequate to establish benefit over standard treatments.

Indications for surgical decompression/fusion

Covered when ALL of the following are met

Indication and conservative care: Patient has symptomatic lumbar spinal stenosis causing significant symptoms or poor quality of life, and symptoms have not responded adequately to first-line nonsurgical approaches (rest, NSAIDs, muscle relaxants, corset use, physical therapy, lumbar epidural steroid injections).failure of appropriate conservative care

Surgical decompression with or without fusion may be indicated for moderate to severe symptoms or poor quality of life after failure of conservative therapy.

Surgical objective and technique: Decompression aims to remove pressure on neural elements (eg, partial/complete removal of lamina, facet joint resection, removal of disc fragments or osteophytes); when fusion (arthrodesis) is performed, appropriate grafting and, if used, instrumentation (screws, rods, plates) should be described in operative plan and documentation.

Procedure description and planned approach should match the clinical indication and imaging findings.

Dynamic stabilization / facet arthroplasty: Current evidence from predominantly nonrandomized series and limited randomized data is insufficient to permit definitive conclusions about long-term benefits and safety of dynamic stabilization systems or facet arthroplasty compared with conventional fusion; preliminary RCT data (2024) report short-term advantages for facet arthroplasty versus fusion but long-term durability and implant longevity remain unknown.insufficient long-term evidence

Facet arthroplasty and dynamic stabilization coverage

Coverage stance and criteria

Facet arthroplasty is not established as beneficial or safe because well‑designed, long‑term randomized controlled trial data are lacking; interim and preliminary reports exist but are incomplete.

Facet arthroplasty is considered investigational/unproven pending substantive long‑term evidence.

Dynamic stabilization (TOPS): The TOPS System (Premia Spine) has received FDA PMA for a motion‑preserving indication following decompression; coverage for TOPS is subject to contract terms and prior authorization requirements, and requests should include device identification and supporting clinical rationale and regulatory information.

FDA PMA status is informational; coverage depends on medical necessity determination and payer contract.

Dividing treatment of symptomatic, multi-site spinal pathology into serial or staged multiple sessions when a single operative session can address all sites is not supported. The policy states that such staged multiple-session approaches are considered unproven and not medically necessary because of insufficient evidence of safety and efficacy.

Splitting spine decompression procedures into serial, multiple, or staged sessions when one session can address all involved sites is not supported by evidence. The policy explicitly indicates there is insufficient evidence of efficacy to justify staged decompression procedures in these circumstances.

There are currently no FDA‑approved facet arthroplasty devices. As stated in the policy, this regulatory status implies that facet arthroplasty devices are considered investigational in the absence of approved devices and adequate supporting evidence.

The policy identifies isolated facet fusion (with or without instrumentation), dynamic stabilization systems for degenerative spondylolisthesis, and total facet joint arthroplasty as unproven and not medically necessary due to insufficient evidence of efficacy.

Dynamic stabilization systems and total facet arthroplasty lack adequate data from well‑designed, long‑term randomized controlled trials.
