Sodium hyaluronate (hyaluronic acid, HA) is a synovial fluid component used as intra-articular viscosupplementation to lubricate joints; commercially prepared products differ by molecular weight and cross-linking and may be derived from bacterial fermentation or avian sources. FDA approvals are limited to treatment of pain due to osteoarthritis of the knee, and no product has consistent evidence of superiority over others.

Systematic reviews and comparative evidence are mixed: ECRI (2019) found evidence inconclusive with some patients getting relief and questions about optimal formulation and populations; Hayes (2018) reported moderate-quality evidence of better function versus saline but inconsistent pain benefit versus corticosteroids at 6 months and no substantive safety differences among products; meta-analyses (Bannuru 2011 and others) indicate efficacy for knee pain with onset by 4 weeks, peak at 8 weeks, and residual effect at 24 weeks though trial quality and heterogeneity limit conclusions.

Randomized trials and RCT-based reviews show variable results across joints: some RCTs and systematic reviews demonstrate modest benefit for knee OA while evidence for hip, ankle, shoulder, and TMJ is limited or inconsistent and generally considered insufficient to establish efficacy for non-knee indications.

Evidence for vocal fold injection laryngoplasty (injection laryngoplasty, IL) with HA is limited but suggestive: several small studies and prospective series (e.g., Wang et al. 2015) report short- to medium-term improvements in glottic closure and voice measures and that many patients required only a single injection; randomized or high-quality comparative RCT evidence is lacking (Cochrane review found no qualifying RCTs comparing alternative injection materials).

Contraindications include known hypersensitivity to hyaluronate preparations or avian-derived products (not applicable to Orthovisc), hypersensitivity to gram positive bacterial proteins (Orthovisc only), and infection or skin disease at the injection site/joint.

Use of HA preparations for skin contouring, acne, scars, and wrinkle treatment is considered cosmetic and not covered, since these indications are not functionally impairing.
