Subacromial balloon spacers (SABS/InSpace) coverage stance

SABS stance: Subacromial balloon spacers for treatment of rotator cuff tears are considered unproven and not medically necessary due to insufficient or low-quality evidence of efficacy and durability; use may be limited to research contexts or evaluated case-by-case in line with external guidance.

Policy cites limitations in existing studies (short follow-up, small cohorts, nonrandomized designs, potential industry bias) and evidence assessments concluding minimal or inconclusive support; NICE recommends restrictions when debridement is suitable and research-only use when not suitable.

Evidence, Regulatory Indications, and Guideline Constraints

Coverage considerations and evidence-based limitations

Evidence summary: Evidence assessments (ECRI 2021, updated 2024; Hayes review 2025) report that InSpace is safe and may improve function and quality of life for large to massive MRCT, but comparative effectiveness versus arthroscopic repair or debridement is inconclusive; included studies report no outcomes beyond two years and have methodological limitations.

Regulatory and indication constraints: The InSpace Subacromial Tissue Spacer System has FDA DeNovo Class II clearance with an indication for massive, irreparable full-thickness rotator cuff tears in individuals >= 65 years with mild to moderate glenohumeral osteoarthritis; intended for patients who may benefit from a shorter surgical time compared to partial rotator cuff repair.age >= 65

Guideline limitation:

Per external guidance cited in the policy, biodegradable subacromial spacer insertion should not be used when debridement is a suitable option. NICE IPG 775 (2023) explicitly states that when debridement is appropriate, spacer insertion for rotator cuff tears should not be performed. When debridement is not suitable, NICE recommends spacer use only in research contexts with multidisciplinary patient selection and surgeons who have specific training in the procedure.

NICE Interventional Procedures Guidance (IPG 775, 2023) advises that biodegradable subacromial spacer insertion should not be used when debridement is a suitable option. If debridement is not suitable, NICE limits spacer insertion to research settings only and recommends randomized trials with patient selection by a multidisciplinary team, surgeon-specific training, and follow-up ideally of at least 2 years.

Subacromial balloon spacers (including the InSpace system) are considered unproven and not medically necessary for treatment of rotator cuff tears because the published literature is limited by short follow-up, small or nonrandomized studies, potential industry bias, and inconclusive comparative effectiveness versus arthroscopic repair or debridement. ECRI (2021, updated 2024) and Hayes reviews conclude that while the device appears safe and may improve pain and function in some studies, the overall evidence is insufficient to demonstrate durable, clinically meaningful benefit compared with established surgical options.

Routine use of subacromial spacer insertion in place of debridement or partial rotator cuff repair is not supported by comparative evidence showing a clinically meaningful advantage. Evidence syntheses note the absence of trials with long-term (>2 years) outcomes and inconclusive results when directly compared with debridement or repair; NICE therefore recommends spacer insertion only within research when debridement is not appropriate and not as a routine alternative to standard surgical options.

Providers must obtain prior authorization for listed shoulder procedures when applicable and follow InterQual CP: Procedures (Arthroscopy or Arthroscopically Assisted Surgery, Shoulder; Arthroscopy, Diagnostic, +/- Synovial Biopsy, Shoulder; Joint Replacement, Shoulder; Removal and Replacement or Revision, Joint Replacement, Shoulder).

Applicable CPT and other billing codes should be submitted with the prior authorization request (see Applicable Codes section).
Prior authorization decisions will reference the InterQual clinical coverage criteria listed above.

Requests for shoulder surgery must include complete clinical documentation demonstrating medical necessity per InterQual criteria. Providers should document the clinical indications, prior treatments tried, and how the case meets the specific InterQual procedure criteria (e.g., indication for arthroscopy vs joint replacement vs revision).

Clinical Background and Scope

Massive irreparable rotator cuff tears (MIRCTs) can be managed with several surgical strategies including arthroscopic debridement, partial repair, or conversion to reverse total shoulder arthroplasty. The InSpace biodegradable subacromial balloon spacer is intended as a minimally invasive option to reduce friction between the acromion and humeral head and to improve pain and function, biodegrading over about 12 months. However, current evidence is limited: systematic reviews, randomized trials, and nonrandomized studies have reported short-term improvements but lack sufficient sample size and long-term follow-up to establish comparative effectiveness versus debridement or partial repair. Guideline and evidence assessments therefore restrict routine use and recommend further randomized research, multidisciplinary patient selection, and attention to device-specific FDA indications (for informational purposes).
