Interspinous fixation (fusion) devices are proven and medically necessary when performed according to FDA labeled indications and all of the following are met:

Interspinous fixation coverage criteria: Back pain of discogenic origin, with degeneration of the disc confirmed by history and radiographic studies; No more than grade 1 spondylolisthesis; Used with an interbody cage as an adjunct to fusion at a single level in the lumbar spine (L1-S1).

Devices must be used according to FDA labeled indications, contraindications, warnings, and precautions.

NASS-based coverage considerations for interspinous devices (static, without fusion) and interspinous fixation with fusion:

Indications for static interspinous distraction devices: Clinically symptomatic degenerative lumbar stenosis with neurogenic claudication relieved by lumbar flexion; can sit at least 50 minutes without pain; can walk at least 50 feet; advanced imaging (CT or MRI) shows mild to moderate stenosis (approximately 25–50% reduction in lateral/central foraminal diameter); failure of ≥6 months of nonoperative treatment; degenerative scoliosis ≤25°; Grade I or less degenerative spondylolisthesis; open decompression (e.g., laminectomy) is not medically safe due to comorbidities; BMI < 40; no prior lumbar surgery at the intended level.

Indications for interspinous fixation devices with fusion: Device used as an adjunct to lumbar fusion procedures (including when used with robust open laminar and/or facet decortication and fusion, autograft inter- and extra-spinous process decortication and fusion, and/or interbody fusion of the same motion segment) for diagnoses such as stenosis, disc herniation, or synovial facet cysts; should not be used without performing open decortication and fusion of posterior bony elements or interbody fusion.

Scenarios where interspinous fixation/devices are NOT indicated: Spondylolisthesis greater than Grade I; bilateral isthmic spondylolysis; significant dynamic instability (e.g., on flexion-extension views); scoliosis or signs of coronal/rotatory instability; fusion indicated at more than one lumbar level; generally not indicated at L5–S1 except in favorable anatomy; complete resection of the facet (unless accompanied by interbody fusion); degenerative spondylolisthesis Grade II or higher; degenerative scoliosis >25°; symptoms not relieved by flexion; patient medically suitable for direct decompression; primarily axial back pain unrelated to activity; BMI ≥40; fixed motor deficit; prior lumbar surgery at the intended level.

Interspinous fixation (fusion) devices are considered medically necessary when implanted in accordance with U.S. Food and Drug Administration labeled indications and when all specified clinical criteria are met. Key covered requirements include documented back pain of discogenic origin with radiographic confirmation of disc degeneration, no more than Grade I spondylolisthesis, and use as an adjunct to fusion with an interbody cage at a single lumbar level between L1–S1.

Devices that provide interspinous decompression or interlaminar stabilization without fusion are considered unproven and not medically necessary for the treatment of spine pain or lumbar spinal stenosis; claims for these indications may be denied. Interspinous fixation devices should not be used in situations identified by NASS and the policy as contraindications, including but not limited to significant instability, multilevel fusion indications, or anatomy unsuitable for safe fixation.

Static interspinous distraction devices are not indicated for degenerative spondylolisthesis of Grade II or higher, degenerative scoliosis greater than 25°, dynamic instability at the operative level, patients whose symptoms are not relieved by flexion, patients medically suitable for direct decompression, those with primarily axial back pain unrelated to activity, BMI ≥ 40, fixed motor deficit, or prior lumbar surgery at the intended level.

Use of interspinous decompression/spacer devices or interlaminar stabilization devices without fusion is considered not medically necessary because the evidence does not demonstrate they are as safe and effective as standard decompressive or fusion procedures. When considering static devices, adherence to NASS criteria is required, including documented failure of 6 months of nonoperative therapy and the specific functional and radiographic thresholds described by NASS.

The body of evidence for interspinous decompression and interlaminar stabilization devices is limited by low-quality studies, risk of bias, and a relative lack of independent randomized controlled trials. Systematic reviews and technology assessments note concerns about study design, small sample sizes, and inconsistent long-term outcomes; some analyses have reported higher reoperation rates with stand‑alone interspinous devices compared with standard decompression, underscoring uncertainty about comparative effectiveness.
