Coverage Criteria for Interspinous Devices

Covered when ALL of the following are met

Interspinous fixation (fusion) devices are proven and medically necessary when performed according to FDA labeled indications and ALL of the following are met:

Use of interspinous fixation devices without interbody fusion (i.e., as a spacer or decompression-only/off‑label use) is not supported by the fusion-device coverage criteria. The policy requires that interspinous fixation devices be used in accordance with FDA labeled indications and as an adjunct to fusion with an interbody cage at a single lumbar level (L1–S1). Off‑label standalone use may increase risk of complications such as spinous process fracture and may negatively affect interbody cage and bone graft mechanics.

Interspinous distraction or fixation devices are not indicated in clinical scenarios that exceed NASS or device-specific anatomic and deformity limits. Examples include: degenerative spondylolisthesis of Grade II or higher, degenerative scoliosis greater than 25°, dynamic instability at the operative level, symptoms not relieved by lumbar flexion, patients who are medically suitable for direct decompression (e.g., laminectomy), primarily axial back pain unrelated to activity, body mass index ≥ 40, fixed motor deficit, or prior lumbar surgery at the intended level. Additionally, interspinous fixation is generally not indicated when fusion is required at more than one level or when anatomy (for example small S1 spinous process) limits fixation.

Interspinous fixation devices are contraindicated or not recommended in situations where adequate fusion or posterior decortication cannot be performed or where specific anatomic/pathologic conditions exist. These include spondylolisthesis greater than Grade I, bilateral isthmic spondylolysis, significant dynamic instability (e.g., on flexion–extension views), scoliosis or coronal/rotatory instability, and cases requiring fusion at more than one level. Use at L5–S1 is generally not indicated because of mechanical limitations of fixation to a small S1 spinous process unless anatomy is favorable.

NASS guidance and the policy emphasize that interspinous fixation devices should be used only as an adjunct to a robust open decortication and fusion or interbody fusion of the same motion segment. There is no literature support for using interspinous fixation without performing an open posterior decortication and fusion or without interbody fusion; decompression‑only or interlaminar stabilization without fusion is not supported by the fusion-device coverage criteria.

The body of evidence for stand‑alone interspinous process devices (spacers/IPDs) is limited and of low to mixed quality. Systematic reviews and trials have reported signals of higher complication and reoperation rates compared with standard decompression, and long‑term comparative evidence is insufficient. Given these findings, stand‑alone IPD use lacks adequate evidence to be considered as safe and effective as standard decompression surgery outside the specific NASS qualifying criteria.

Lumbar spinal stenosis is narrowing of the lumbar spinal canal that can compress nerve roots and produce back and leg pain, neurogenic claudication, and in severe cases neurologic deficits. Clinical presentation commonly includes buttock, thigh, or leg pain and symptoms that are relieved by forward flexion. Management options range from conservative care to decompressive surgery with or without fusion; interspinous devices have been developed as less invasive alternatives, with distinct device classes (rigid interspinous fixation devices used as fusion adjuncts versus interspinous distraction/spacer devices used for indirect decompression).

Conservative Therapy Expectation

Conservative therapy expectation. For interspinous distraction (decompression) devices intended instead of direct decompression, documentation should show failure of an adequate course of nonoperative treatment prior to consideration — typically at least 6 months unless contraindicated. Use of decompression-only or interlaminar stabilization devices without fusion is at higher risk for denial if conservative therapy criteria and imaging/clinical correlation are not documented.
